General Position Description

The Outreach Case Managers will work in teams of two, to engage people living in unsheltered locations such as cars, parks, abandoned buildings, encampments, and the streets. Areas of service to be determined by the City of Eugene, County, and the outreach teams. These teams will provide essential services to the unsheltered single adults sleeping in places not meant for habitation with the goal of helping people in addressing immediate health and safety needs, assist in accessing benefits, and assist in coordinating housing support services with a primary focus on obtaining stable permanent housing. The team will also collaborate with community, city, and county partners to best meet the needs of this population.

Responsibilities

  • Carry a client load of outreach clients, provide assistance, advocacy, and appropriate referrals to unhoused clients
  • Perform client assessments and assist with goal development
  • Complete Front Door Assessor training to perform FDAs in the field
  • Provide outreach and case management in the field
  • Help clients engage with community resources
  • Track all encounters in HMIS street outreach including entry and exit, UDS, and EHR
  • Track and record statistics for required reports
  • Load outreach van at the start of shifts and unpack after the shift
  • Keep outreach vehicle clean and in good working order
  • Attend program meetings and outreach case conferencing meeting
  • Participate in New Bird Training

Required Qualifications

  • At least 2 years’ experience in a crisis intervention, mental health services, substance abuse services, or other related field
  • Ability to work comfortably and effectively with diverse populations, especially the unhoused
  • Possess a valid Oregon drivers license and the ability to be insured under White Bird Clinic’s vehicle insurance
  • Must be able to lift 50 pounds
  • Able to stand for extended periods of time and walk up to 2.5 miles in a day
  • Ability to network with other agencies and assist with Government resources
  • Ability to work on the street and other non-traditional environments, as well as an office setting
