Job description

Nurse Practitioners needed for addiction and treatment center with locations in Brooklyn and Harlem.

REPONSIBILITIES:

• Speaking with and staying in contact with Department of Health regarding specific patients
• Reviews patient’s past medical history and formulates a list of current and past medical conditions using clinical knowledge and judgement and document such findings in the EMR.
• Reviews current symptoms and identify diagnoses to be used in care management and active medical management of treatment
• Review’s medication review
• Review of lab results within the medical center
• Check’s vitals, conducts a physical exam (wellness exam as well when appropriate) that follows clinical protocols established by ParCare Community Health Network
• In complex cases, interacts with the physician to review and collaboratively address appropriate care
• Educates patient and family members of the disease process, medication administration and compliance need of the care plan
• Comply with all HIPPA regulations and maintain security of protected health information (PHI)
• Refers patients to Physician for consultation or to specialized health resources for treatment.

REQUIREMENTS:

• Masters in Science - Nurse Practitioner
• NYS Licensed Nurse Practitioner
• Prior experience in Family Health, Internal Medicine, Behavioral Health, or Addiction

Job Type: Full-time

Pay: $125,000.00 - $140,000.00 per year
