HOME CARE PROGRAM ASSESSOR
This position will have daily travel utilizing a company car. There is no on-call and the schedule is Monday-Friday with no weekend work. Employee may live anywhere within the program counties but must be willing to travel daily to scheduled assessment home location to meet with individual in their home location/nursing home
This position is remote based home office with home assessment visits within the our OHC program counties. Focused travel within Allen, Putnam, Hancock, Hardin, Van Wert, Mercer, Logan, and Auglaize. But, could potentially travel within Darke, Shelby, Champaign, Miami, Preble, Montgomery, Greene, Warren, Clinton, and Preble for needed coverage.

- Monday: 2 assessments
- Tuesday: 2 assessments
- Wednesday: 1 assessment
- Thursday: 2 assessments
- Friday: 1 assessment
Home office with provided equipment. Minimal visits to home office in Lima, Ohio for meetings/events.
This is an assessment position only with no hands on nursing.
MINIMUM QUALIFICATIONS:
- Current RN or LSW licensure in the State of Ohio.
- Bachelor’s degree in Human or Social Services such as psychology, social work, sociology, or counseling
- Must have a valid driver's license, access to an automobile, and automobile insurance meeting the minimum requirements of state law.
- Proficient in computer skills.
- Must have a telephone.
- Final candidate must agree to a BCII Criminal Records Check, which includes fingerprinting and pre-employment Drug Screen at the expense of Area Agency on Aging 3.
- It is the sole responsibility of the employee to keep up with all required licensure (Examples
RN, LSW), including continued licensure renewals throughout your employment with Area
DESIRED QUALIFICATIONS:
- Bilingual (Spanish/English) fluency.
ESSENTIAL FUNCTIONS:
- Conducts a face-to-face comprehensive assessment with the individual/family system as a basis for determining the consumer's level of care/level of need, eligibility for available community resources and long-term care options.
- Assessor needs to be trained and proficient in both home care programs. (Ohio Home Care Waiver and Specialized Recovery Services) Annual testing of the ANSA is required with a passing score to remain an Assessor for the Specialized Recovery Services program.
- Assessor will have an agency vehicle pending company insurance coverage. Pending approval, the Assessor is responsible to maintain vehicle standards as defined in Company
Vehicle policy.
- Assessor can travel within all counties covered by the Ohio Home Care Program and Specialized Recovery Services service area
- Assessor can perform Adverse Level of Care Assessments as needed within the Ohio Home Care Program for any Case Manager that is an LSW working within the Ohio Home Care
Waiver program as needed.
- Develops an individualized plan with the individual caregiver which addresses the problems identified, the goals and outcomes to be achieved, the services to be pursued in support of goal achievement, the responsibilities of the consumer, informal/formal support and the case manager and defines payment sources for needed services.
- Arranges for service delivery through a process which includes collaborating and negotiating with service providers (including informal caregivers, volunteers and outside resources) for delivery of needed services to the consumer in a manner described in the care plan.
- Clarifies for individuals/caregivers their community resources and available options prior to
Ohio Home Care Waiver enrollment.
- Links consumers to resources, services, and opportunities that maintain the greatest degree of consumer independence.
- It is the standard that documentation is submitted to supervisor/IE for review no later than 24 hours after assessment is completed.
- Participates in the Area Agency on Aging 3 internal quality assurance and utilization review process as requested.
- This position is directly responsible to the OHC/SRS Supervisor/Vice President of Program Operations/Chief Operations Officer/Chief Executive Officer.
- Performs other related tasks as assigned by the VP Program Operations
- Adheres to consumer privacy and HIPAA regulations.
REQUIRED TRAININGS:
- Trained in all agency computer software systems.
- Attend any training required by Supervisor.
