HealthCare Support is seeking a Remote Oncology Case Manager – RN to join a well-known managed care organization in the state of New Jersey.

Must have an active RN license in the state of New Jersey. Must reside in New Jersey to apply.

Daily Responsibilities:

• The RN Case Manager will provide telephonic case management services to high-risk members
• Develop care plans in coordination with providers, patients, families as appropriate and assess the effectiveness of services
• Contact members of the medical team to discuss the patient’s course of progress and needs

Requirements for the Remote Oncology Case Manager - RN :

• Must have active/clear RN license in the state of New Jersey
• Must reside in the state of New Jersey
• 3+ years of clinical experience as an RN
• Experience working with oncology patients a MUST
