Healthy Families is a home visiting program for expectant parents and parents of newborns experiencing stressful life situations. The HF program improves childhood outcomes and increases family self-sufficiency by empowering parents through education and community support. Parents voluntarily participate in Healthy Families so they can learn how to recognize and respond to their babies' changing developmental needs, use positive discipline techniques, cope with the day-to-day stress of parenting in healthy ways, and set and achieve short- and long-term goals.

Essential Functions: (Duties and Responsibilities)

  • Contact assigned families to discuss program and schedule enrollment if family is interested.
  • Conduct in-home visits with families.
  • Conduct an initial assessment with family and document appropriately the information gathered using the HFM outline.
  • Assist parents in identifying their strengths and needs, and in improving their skills to optimize a stable home environment.
  • Assist parents in improving the family support system.
  • Assist in increasing the family’s ability to problem-solve and advocate for themselves and their children.
  • Identify and refer family to other supportive agencies when appropriate.
  • Assist family in establishing goals and a plan for accomplishment of those goals, as well as the assessment of the normal growth and development of the target child.
  • Track immunizations and well child checks.
  • Complete developmental screenings and provide intervention with the appropriate developmental activities necessary to address possible developmental delays.
  • Complete other assessments/tools as required by the program model.
  • Assist family in strengthening the parent-child relationship.
  • Monitor the well-being of children in the home and report any concerns to supervisor.
  • Document accurately and in a timely manner in participant files all contacts with or on behalf of participant families.
  • Complete forms and paperwork in a timely and comprehensive manner as required.
  • Be prepared for supervision and participate in a punctual, professional and contributory manner.
  • Perform other duties as assigned by supervisor.
  • Participate in agency activities as required.
  • Adhere to all agency policies and procedures.
  • Adhere to all Healthy Families Florida/Healthy Families Manatee policies and procedures.
  • Attend all meetings as required in a punctual, professional and contributory manner.
  • Notify supervisor immediately of any safety concerns

Education and Experience Requirements:

  • High School (H.S.) Diploma or General Education Development (GED) and a minimum of two years of experience working with children age 0 -5 in a professional role.

Specific Knowledge, Skills, and Abilities Required:

  • Must have excellent oral and written communication and organizational skills.
  • Must have demonstrated skills working in a Microsoft Windows environment and be able to use the Agency software within one month of hire.
  • Must have the ability to read and interpret documents such as safety rules, operation instructions, and procedure manuals.
  • Must be sensitive to the needs of the low-income community, demonstrate culturally competent skills, including an understanding of the impact of poverty on families.
  • Must be highly organized, accountable, and able to work independently.
  • Must be able to establish rapport with staff and families and empower them by building on their strengths.
  • Must be able to work in a collaborative, creative, problem-solving environment with major emphasis on program outcomes and family support.
  • Ability to write routine reports and correspondence.

Licenses/Certificates/Screenings:

  • Must have a valid Florida Driver’s License.
  • Must have reliable transportation for work-related travel, including nights and out of town.
  • Must be insurable under Step Up Suncoast’s organizational policies.
  • Pre-employment drug screening required.
  • Must pass a Level II criminal background check.
  • Must agree to ongoing random drug/alcohol screening.

  • While performing the duties of this job, the employee is regularly required to talk or hear. The employee frequently is required to stand; walk; use hands to finger, handle or feel; and reach with hands and arms.
  • Must be able to lift and carry 20 pounds

Expected hours of work:

  • Hours of work and days are typically Monday through Friday, 8:00 am to 5:00 pm., however, you may be called upon outside these hours to complete home visits or attend other functions of the position or agency as needed.
