JOB SUMMARY

The Patient Care Navigator supports the SCMG Care Coordination team in accordance with the philosophy of Swedish Covenant Medical Group by providing administrative and clinical support for our patients. The mission and goal of care coordination is accountable for providing high quality, individualized, goal-directed care for all levels of patients. The Patient Care Navigator works as part of a “shared goal model” team to provide health services to individuals of all age groups.

Following established goals and protocols, this role will conduct outreach efforts to include telephone/ in-person follow up communication to patients, encouraging and promoting annual preventive care, chronic disease management and transition of care. S/he completes wellness and disease-specific templates and communicates identified concerns or needs voiced by patient or family prior to and during visits. As a member of the team, supports the goal to ensure seamless transition of care by encouraging patients to have annual wellness visits, identifying gaps in care with the collaboration of the primary care provider and engaging patients and families in addressing chronic health conditions.


RESPONSIBILITIES

Essential Functions

  • Demonstrates a commitment to the mission of Swedish Covenant Hospital and Swedish Covenant Medical Group and demonstrates a service orientation and adheres to all responsibilities and standards.
  • Clinical skills to include point of care testing (measurement of glucose, rapid strep and influenza) immunizations, measurements of vital signs including blood pressure, temperature, respirations, height, weight, pulse oximetry and waist measurements in all age groups.
  • Facilitates patient access to appropriate medical and specialty providers as necessary to include consultations and testing.
  • Assists and serve as resource to medical practices and referral teams when needed for prior authorization of tests and medications.
  • Serves as patient advocate in both navigating care and follow-up outreach efforts.
  • Participates regularly as part of the care coordination team in meetings and huddles.
  • Participates in quality studies and data gathering as indicated by patient need.
  • Ability to work effectively with culturally diverse populations, non-English speaking and high-risk populations.
  • Performs other duties as assigned.

QUALIFICATIONS/BASIC JOB REQUIREMENTS

  • High school graduate required
  • Graduate of a medical assistant program preferred
  • Two or more years of Medical Assisting work experience required, preferably in adult medicine and QI reporting.
  • One year of EMR experience documenting in electronic health record (e-Clinical Works preferred)
  • Possesses knowledge of age-specific preventive health requirements for all age groups.
  • Strong interpersonal and communication skills
  • Familiarity with preventive care, quality studies and diabetes management
  • Strong patient and team-focused commitment

LICENSE/CERTIFICATION

  • Current Certification or Registration as a Medical Assistant required
  • Current CPR certification
