Patient Navigator- Greenwood

PRIMARY PURPOSE: To assist patients in obtaining services up to and including screenings, completing applications to determine income and the nominal fee based on the sliding fee scale. Assist in outreach and enrollment activities as assigned.


ESSENTIAL JOB FUNCTIONS:

  • Completes and documents patient household income per policy and procedures governing Federally Qualified Health Centers
  • Document and educate patients on insurance status or eligibility for health coverage programs
  • Assists with manual and online applications with patients to obtain health care coverage
  • Assess eligibility for pharmaceutical drug assistance programs
  • Coordinate and complete applications for patients for drug assistance programs
  • Post daily payments
  • Assist with patient demographic data input and quality assurance
  • Public presentation of FoundCare Health and eligibility assistance programs.
  • Distribute health center and insurance eligibility materials. Meet grant funded objectives.
  • Keep records of assigned cases, and prepare required reports.
  • Assists with patient registration as needed
  • Perform other duties as assigned.


REQUIRED KNOWLEDGE, SKILLS AND ABILITIES:


  • Knowledge of insurance programs. Knowledge of principles and processes for providing customer service. This includes customer needs assessment, meeting quality standards for services, and evaluation of customer satisfaction.
  • Ability to orally communicate effectively with others, with or without the use of an interpreter.
  • Ability to communicate effectively in writing using the English language, with or without the use of auxiliary aids or services.
  • Ability to review, understand, and apply concepts presented in training programs, conferences, and/or professional literature.


PHYSICAL REQUIREMENTS:


  • Ability to endure short, intermittent, and/or long periods of sitting and/or standing in performance of job duties.
  • Transport him/herself to other departments, conference rooms, and on occasion, to off-site locations to accomplish job responsibilities, attend
  • Accomplish job duties using various types of equipment/supplies, e.g. pens, pencils, calculators, computer keyboard, telephone, etc.


MINIMUM QUALIFICATIONS:

  • High school diploma / GED
  • Requires training in vocational schools, related on-the-job experience, or an associate's degree
  • Ability to interact and work with diverse populations
  • Knowledge of community insurance programs
  • Previous experience in a patient care setting
  • Possess superior organizational, interpersonal, written/verbal communication skills.
  • PC Proficient
