JOB SUMMARY

The Patient Scheduler works under the supervision of the Clinical Director. The primary function of this position is coordinating the delivery of Centralized Scheduling services for all patients receiving care from Lower Cape Fear LifeCare staff. The Patient Scheduler will be responsible for entering discipline specific scheduled visits for all patients; to include the RN Case Managers, LPNs, Social Workers, Certified Nursing Assistants, Chaplains, and other designated staff as deemed necessary. The Patient Scheduler must associate and assign employees to patients for scheduling purposes. Responsible for communication on a high level with all staff and patients involving patient care services. Responsible for the delivery of scheduling services by assigning and prioritizing tasks. Accountable for quantity and quality of work in the Central Scheduling Department. Performs all Central Scheduling functions. The Patient Scheduler will possess a good working knowledge of the Allscripts Product. Provide backup coverage as needed in the Resource Center. Required to perform all duties in a highly professional manner, demonstrate behavior that exemplifies Lower Cape Fear LifeCare mission and values. Performs other appropriate duties as assigned.


PRIMARY JOB DUTIES

1. Must ensure that all active LCFL patients have accurate schedules for all disciplines involved in providing services, including finding coverage when staff is out.

2. Must maintain and modify schedules on a daily basis

3. Must communicate all high level service or order changes regarding patients.

4. Must ensure all visits are assigned to appropriate staff and cancellations are communicated to the RN Case Manager and employees on the case. Responsible for notification to patient with any schedule changes.

5. Must document any modifications to patient schedules as a note on that visit

6. Must add administrative time (for training, supplies, mileage, etc) as needed to schedules as well as PDO for aides.

7. Must run field status review report (census changes) several times daily, for each business unit.

8. Needs to verify mobile services (for aides) each day, run unverified services report, and correct any conflicts.

9. Coordinate with HR to have contract staff information provided for set up in Allscripts. Assign contract staff to appropriate patient in Allscripts. Verify invoices prior to sending upstairs for approval.

10. Tasks to be completed daily include running scheduling queue task report daily and reassigning tasks to appropriate scheduler.

11. Completes patient scheduling services accurately as ordered.

12. Possesses excellent communication and organizational skills.

13. Maintains and promotes internal and external customer satisfaction.

14. Maintains and adjusts schedule to enhance program performance.

15. Demonstrates positive interpersonal relations in dealing with all members of the team.

16. Effectively demonstrates the mission, vision, and values of Lower Cape Fear LifeCare on a daily basis.


JOB SPECIFICATIONS

1. Education: High school diploma or an equivalent. Associate’s degree or equivalent combination of education and experience.

2. Licensure / Certifications: Not Applicable.

3. Experience: Minimum of one year of phone skills, sales and/or customer service background.

4. Essential Technical / Motor Skills: Must be able to read and interpret technical and financial report/documents. Verbal, written and computer skills necessary to communicate with internal and external customers. Must be literate in English language.

5. Interpersonal Skills: Ability to develop positive interactions with patients, visitors,

physicians, staff, vendors and other external customers to effectively care for patients and manage the operations of the facility.

6. Essential Physical Requirements: Sitting for deskwork and meetings, writing, walking and standing, and climbing stairs.

7. Essential Mental Abilities: Must be able to read and understand directions, policies, procedures, etc.

8. Essential Sensory Requirements: Ability to use sight to perform clerical duties. Ability to use hearing to communicate with staff and patients.

9. Exposure to Hazards: May encounter exposure to infectious disease or situations which present a potential threat to personal safety.

10. Other - Hours of Work: Monday through Friday (8:00am – 5:00pm). May involve calls after hours or on weekends.

11. Population Served: All patient populations from infants through geriatric with life limiting illness.
