POSITION DESCRIPTION: Under direct supervision, the Peer Advocate will provide direct and indirect support services to homeless, co-occurring individuals in providing support and assistance with the development of independent living skills, case management, recovery groups and Client Coordinated Care Plan. Assist by providing linkage / referrals and follow up supportive services.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Be part of assertive assessment and evaluation team to individuals with co-occurring disorders and provide rehabilitative supportive services as needed.
Consult with referring agencies to coordinate services and provide continuity of care and assist team in linking client to appropriate resources.
Facilitate groups addressing recovery, relapse, homelessness and issues of independent living skills and developing and maintaining healthy support systems; provide rehabilitative supportive services as needed.
Assist with Milieu preparation and activities, including: preparing sack lunches, preparing shower and laundry facilities, assisting clients with accessing these facilities, and supervising clients using the Milieu.
Assist designated staff in providing crisis intervention; intervene at the onset of a crisis to provide support and assistance in problem resolution.
Transport clients to and from appropriate services as necessary.
Facilitate back-up groups for treatment staff on dual diagnosed issues.
Serve as role model for clients- assist clients in developing and maintain daily living skills needed to implement and achieve the goals of the individual service plan.
Complete all documentation within mandated time frames and in compliance with county, state and Center guidelines.
Will work as part of multi-disciplinary team, participate in staff and clinical meetings, attend training as scheduled, and other meetings as assigned.
Will perform other duties as assigned.
MINIMUM REQUIREMENTS:
Eighteen (18) months of mental health stability and stable on medication, (if prescribed).
Twelve (12) months of successful recovery from substance abuse and participation in on-going self-help groups.
Must have a valid California Driver License and be insurable by the Center’s auto insurance company and maintain a good driving record.
Knowledge and experience in working with co-occurring diagnosed CMI population preferred.
Arriving at work on time and not leaving early is an essential function of the job.
