Hackley Community Care (HCC) is a Federally Qualified Health Center that offers integrated health services such as dental, medical, behavioral health, and pharmacy to the residents of Muskegon County. HCC is located along the shores of Lake Michigan and Muskegon Lake where people come to Work, Play and Stay.

The Pharmacist will provide pharmaceutical care to HCC patients and residents, including processing and accurately dispensing prescription orders, providing education regarding health care and prescription medication needs, maintaining confidential information, maintaining controlled medication and required documentation.

Job Qualifications:

• Graduated from an accredited pharmacy school program.

• Current active, in good standing, licensures to practice pharmacy in the State of Michigan.

• Knowledge of pharmacy laws.

• Evidence of professional growth (CME’s/CEU’s) consistent with licensure requirement for State of Michigan.

• Maintain BLS certification.

• Certified to give immunizations (preferred).

• Willing and able to work under collaborative practice agreement and comfortable interchanging prescribed medication(s) via organizational formulary.

• Provide patient focused care with strong interpersonal communication skills.

• Competent in pharmacy billing and utilizing resources to determine formulary coverage.

• Willing to work closely with providers to establish and execute a patient care plan.

• Able to give providers clinical recommendations based on current clinical guidelines and patient specific factors.

• Experience documenting within an electronic medical record (preferred).

• Experience with Microsoft Office is preferred (or willing to learn).
