A. Primary Purpose of Organizational Unit:
The Community Alternatives Program is a Medicaid funded service which allows individuals to receive services in their home to avoid long-term care.
B. Primary Purpose of Position:
To provide nursing assessments for the Haywood County Health and Human Services Agency Community Alternatives Programs (CAP/DA, CAP/C).
This position will provide nursing assessments for the Community Alternatives Programs (CAP) Program per program requirements. Provide general health clinic services and education to clients served. This nurse will:
Have knowledge and understanding of the physical, mental and emotional aspects of aging, the resultant diseases and infirmities and related medications and rehabilitative measures
Conduct physical assessments of clients
Assess and educate clients about basic care, skills, and abilities
Connect families with community resources
Establish and review health policies and procedures
Coordinate with community based health care providers
Performs other duties as assigned
C. Work Schedule:
Average of 30-40 hours per month depending on the number of assessments and re-assessments to be completed.
These times may vary depending on the day to day tasks. Other hours may be set or required per the County policy. Description of Responsibilities and Duties
100% CAP Nurse:
Community Alternatives Program medical assessments:
Coordinate with CAP case managers during assessments to qualify individuals for the services and re-assess as required by program guidelines.
Complete necessary health assessment forms
Note: This position may be called upon to serve in a shelter during a county emergency or to occasionally serve in Haywood County Health and Human Services clinics if called upon due to special circumstances.
Also, may have other duties as assigned. Qualifications
Graduation from a four-year college or university with a BS Degree in Nursing which includes a public health nursing rotation and one year of public health nursing experience; or graduation from an accredited school of professional nursing and two years of professional nursing experience including one year in public health; or an equivalent combination of education and experience.
Additional Training/Experience:
Adult Physical Assessment HIV Training
CPR & First Aide Certification annually
License or Certification Required by Statute or Regulation:
Licensed to practice as a Registered Nurse in North Carolina by the NC Board of Nursing
Knowledge, Skills and Abilities
Special Requirements - Considerable knowledge and skill in applying nursing practices, principles, and techniques. Needed is a working knowledge of current social and economic problems related to public health. Knowledge of resources and organizations. Ability to secure cooperation of clients; deal tactfully with others, use good judgment in assessing situations and making decisions. Ability to plan and coordinate daily activities and supervise the work of others is a necessary skill. Physical Requirements
Position requires mostly sedentary work that will occasionally require walking or standing for brief periods of time.
Requires exerting up to 10 pounds of force on a recurring basis as well as routine keyboard operations.
The job requires normal visual acuity and field of vision, hearing and speaking ability.
The position risks no exposure to environmental hazards.
Physical Effort:
Great physical effort is not necessarily a part of the job, but long hours to attend evening meetings or to work on agency related matters may be required. Occasional travel out of town may also be necessary. Stamina is required to endure and resolve crisis oriented work situations.

ADDITIONAL REQUIREMENTS FOR POSITION:
Applicants must submit official college transcripts (if applicable). Applicants must have NC Driver"s License and access to a vehicle. The applicant must have a clean driving record with no more than three (3) points showing on current DMV record.
