Physical Therapist (IDA)
Job description
Position Summary: The Physical Therapist will conduct evaluations of children who may have developmental delays. The purpose of the evaluation is to determine eligibility for CA Early Start services.
Essential Functions and Responsibilities:
- Communicates with assessment partner and family to confirm appointment.
- Completes Infant and Toddler Developmental Assessment and intake paperwork.
- Completes M-CHAT review if autism spectrum disorder (ASD) is suspected.
- Reviews SDRC materials with family during assessment.
- Obtains and reviews vision/hearing screening forms.
- Develops report with a descriptive summary of the childs developmental competencies that include a performance age range.
- Assists in developing relevant portions of Individual Family Service Plan (IFSP).
- Prepares and completes the Provence Profile (IDA) and all intake forms for submission to supervisor within 72 hours of IDA completion.
- Maintains an IDA kit and assessment materials.
Education & Experience:
- Experience providing services for or with children between the ages of four months to 3 years of age with established risk or those at-risk for Intellectual Disability / Developmental Disability
- Masters Degree in Physical Therapy from an approved program by The Commission on Accreditation in Physical Therapy Education (CAPTE)
- Current Physical Therapy license from the Physical Therapy Board of California
Technical/Functional Skills:
- Read, write, and understand the English language.
- Bilingual staff may interpret for families and other staff.
- Proficient in Microsoft Word and Outlook
- Proficient on office equipment including computer, printer, and copy machines.

- Ability to form positive, supportive, and professional working relationships
- Detail-oriented with ability to ensure accuracy of program documents and paperwork
- Ability to manage work time efficiently and meet deadlines without close supervision

Minimum Requirements:
- Must be 18 years of age or older at the time of hire
- Must receive criminal record clearance from DOJ
- Must have valid California Drivers license, safe driving record, and have been licensed to drive at least 2 years
- Must maintain current and adequate auto insurance
- Must obtain CPR/FA certification if not already certified
- Must be fully vaccinated for COVID-19 (including required booster shots) or have received an approved exemption from HGH by date of hire
Physical Demands/Environment:
Able to see, talk, hear, walk, bend, stoop, sit, stand, reach, and lift items weighing 10 pounds or less repeatedly during a shift.
Job Type: Part-time
Pay: $50.00 - $60.00 per hour
