Commonwealth Care Alliance’s (CCA) Community Physical Therapist (PT) is primarily responsible for providing short-term in-person physical rehabilitation care to a flexible panel of dually eligible CCA members, a group of individuals with significant medical, behavioral and social complexities that require intensive clinical support. The Community PT is deployed by a member’s Care Partner or via a referral from others, including the Transition of Care team, community providers or home care agencies to provide short term, rehabilitation care and consultation to the member and care team on decision making regarding skilled rehabilitation services. The Community PT also monitors falls, fall risk, functional decline, pain, caregiver training, member training & education, durable medical equipment (DME) assessments & authorizations, discharge planning & meetings, wound care, and aftercare following emergency department and/or skilled nursing facility admissions, and authorization review for skilled services in a long-term care (LTC) setting. The Community PT reports to the Manager, Community Rehab.
- Master's Degree in Physical therapy preferred - Bachelor’s degree with 10 year’s clinical experience preferred - Must be licensed as a Physical Therapist in Massachusetts, in good standing - 5+ years of clinical experience with a focus on geriatric populations, home care, disability, short term rehabilitation, managed care or a combination of the above - Must be licensed as physical therapist in Massachusetts in good standing - Requirements include: - Ability to utilize an Electronic Medical Record - Ability to use on-line training platforms - Demonstrated understanding of the Model of Care - Demonstrated understanding of the benefits of each program - Ability to review welcome packets and obtain consent forms and attach them to EMR - Ability to complete and update a Care Plan that meets CCA requirements - Demonstrated understanding of LTSS - Demonstrated understanding of how to use CDSTs when ordering services - Ability to complete and lock all required notes and telephone encounters within 48 hours - Demonstrated understanding of Referral to Specialists - Ability to create referrals and authorize services within appropriate time frames - Fluency in English required; fluency in one or more of the following languages strongly preferred: Spanish, Portuguese, Russian, Nepalese, and Vietnamese. - Experience with electronic medical record strongly preferred (eCW a plus)
- Conducts Physical Therapy Assessments and Treatments administered as indicated. - Monitors Fall Log for One Care and SCO members, as requested by Care Partner or community care team, and initiates fall response, as needed (recognizes trend and provide most appropriate therapeutic intervention as indicated). - May visit members in Skilled Nursing Facilities (SNF) and observes their mobility & rehab sessions; consults with rehab team, social worker and case managers in collaborating member’s skilled services and discharge plans. - Evaluates legitimacy and coordinates cost effective solutions for DME requests and authorization review. - Responds to member-directed interventions and all communication. - Conducts home safety assessments and makes modifications as needed. - Works with CCA hospitalists and community providers in assessing and decision making of client’s skilled rehab and discharge plans. - Participates in joint visits with Care Partners and other Community Team members, as needed, to collaborate and develop most effective plan of care. - Documents all activities and results using an Electronic Medical Record, in an effective manner while strictly adhering to CCA policies and procedures - Updates the member care plan to include appropriate rehab goals and interventions. - Seeks additional training from the Clinical Education Team, Preceptor or other clinicians at CCA to obtain the competency needed for duties and responsibilities. - Attends all mandatory meetings and completes all mandatory trainings, including but not limited to LMS and in-person trainings.
