ABOUT OUR TEAM
The Program Manager ensures the delivery of high-quality supportive services to the tenants at Vela Apartments, a housing development supporting formerly homeless families. Under the guidance of Associate Director and PATH leadership, this position provides leadership and direct supervision to the onsite service team, oversees operational compliance for the programs and serves as a representative of the program both internally and externally.

WHAT IS A PROGRAM MANAGER?
As part of the San Jose Team, the Program Manager performs evidence-based practices, such as Housing First, Motivational Interviewing, Critical Time Intervention and Non-Violent Crisis Intervention, within the programs by providing direct client services to a small case load and training staff on these practices. Position Responsibilities:

  • Program Management & Oversight- In collaboration with the Associate Director and Director of Programs, provide administrative oversight of the contract to ensure successful implementation, coordination of participant services, achievement of program performance measures, and evaluation of services provided. Ensure the program is in compliance with contracts, partnership agreements, program goals, stakeholder expectations, and all other funding requirements. Operate as the primary contact person for all program matters. This includes program monitoring by funders, responding to specific program inquiries from outside partners, collaborating with other PATH programs, and working with the Finance department to monitor program budgets. Ensure all documentation is accurately entered into required program documentation systems as contractually required.

  • Clinical oversight- Provide clinical support, training and supervision to program staff. This position is expected to be available for clinical consultation, provide regular supervision, and complete assessments and treatment plans. Be knowledgeable and have experience providing evidence based practices effective with a vulnerable homeless population including but not limited to, Motivational Interviewing, Critical Time Interventions, Harm Reduction, Housing First and Crisis and Suicide Assessment.

  • Staff Support/Development- Provide direct supervision to all direct reports. Plan and schedule on-going workload assignments set and adjust work to be accomplished by staff; and prepare schedules for work completion. Conduct weekly supervision meetings with staff; provide regular and appropriate feedback
    including training, opportunities for professional growth, verbal and written improvement plans when needed, and regular evaluations.

  • Program Outreach – Educate, network and represent programs to local agencies, coalitions, community stakeholders, Law Enforcement, City and County government, residents, and other organizations.

  • Reports/ Contracts and Compliance – This position is expected to work autonomously and conduct critical data analysis to evaluate and improve service delivery. Review and authorize all program and contract reports prior to submittal. Review and explain contract scope and services to staff. Submit regular monthly report. Work in collaboration with the Quality Assurance Department to perform regular internal audits. Attend all monitoring site visits.

  • Supportive Services – Provide case management services to a small caseload of more challenging and complex tenants. Meet tenants at least twice per month to provide strengths based case management and service coordination services designed to assist tenants in maintaining stable housing. Provide crisis intervention services focused on enhancing the tenant's ability to independently problem solve, utilize effective coping skills, and coordinate own care. Facilitate on-site groups related to topics that enhance life skills, health and wellness, and housing stability

WHAT YOU BRING
We're looking for candidates that:

  • Knowledge and understanding of Permanent Supportive Housing evidence-based practices such as intensive case management, Motivational Interviewing, Harm Reduction, Critical Time Intervention and Housing First practices
  • Experience in homeless services, case management, mental health and permanent supportive housing
  • Excellent verbal and written communication skills.
  • Strong organizational, planning and time management skills.
  • Excellent interpersonal skills with the ability to work in a variety of settings with culturally diverse persons and communities with the ability to be culturally sensitive and appropriate.
  • Strong ability to effectively resolve and cope with immediate conflict and/or crisis situations.
  • Proficient use of computers, basic office software and any other database software(s) used to track work outputs.
  • Excellent leadership skills with a hands-on, lead-by-example work style.
  • Knowledge of community resources.
  • Ability to maintain and execute confidential information according to HIPAA standards.
  • A high level of tolerance and understanding for individuals who present for services with urgent multiple case management and health needs.
  • Ability to work independently and within a team environment and exercise mature judgment.

PREFERRED QUALIFICATIONS

  • MSW/MFT/MA strongly preferred.
  • Two (2) years of experience in a leadership role

MANDATORY REQUIREMENTS
Regardless of qualifications, candidates must:

  • Have employment eligibility verification
  • Have or be able and willing to obtain CPR/First Aid training
  • Provide proof of full COVID-19 vaccination
  • Successfully complete the following as a condition of hire:
    • Tuberculosis Test
    • Background Screening
    • Drug Test

Driving is an essential function of this position. To meet the employment requirements for this role, all candidates must:

  • Have reliable transportation
  • A valid driver's license
  • Proof of insurance and ownership for personal vehicles used during work duties
  • The ability to qualify for PATH's insurance coverage

COVID-19 VACCINATION
Effective 11/01/2021 ll require all employees to be fully vaccinated for COVID-19. All prospective hires will be expected to provide proof of full vaccination as part of the pre-employment credentialing process.
