Orange County Program of All-Inclusive Care for the Elderly (PACE) is dedicated to providing its participants with comprehensive health and social supports that are proven to effectively manage chronic conditions and to reduce the risk for premature institutionalization. Responsibilities:
The Registered Nurse (RN) – Level of Care (LOC) is responsible for assessing, coordinating, monitoring, and providing health care services for the Innovative Integrated Health, Inc. (IIH) participants.
Provides detailed medical history and lifestyle assessment to applicants who are enrolling in IIH PACE both in-home and the PACE center. Responsible for data entry and scanning documentation into the electronic health record. Responsible for writing assessment summary and narratives for State review. Coordinates communication between IIH PACE and Department of Health Care Services (DHCS). Organize and submit reporting information to DHCS such as but not limited to first submission, resubmission, or deferral status for applicants or prospective enrollees. Coordinate with the Intake Staff Master’s Level - Social Worker (MSW), or Primary Care Provider (PCP) if additional applicant information is required. Monitor the status of DHCS submissions and coordinate with Intake Staff. Responsible for Home Safety Assessments when necessary. Coordinate with License Vocational Nurse (LVN) - Home Care Coordinator when reassessments are required for applicants. Responsible for creating initial care plans and collaboration with IDT during care planning or as soon as participant is enrolled. Coordinate with upcoming appointments with scheduling and DME vendors to return rental DMEs to not contracted vendors and order a replacement DME from contracted vendors such as but not limited to oxygen, wound vac, hospital bed, and Maintain confidentiality of applicant information. Attend and participate in staff meetings, in-services, projects, and committees as assigned. Adhere to and support the center’s practices, procedures, and policies including assigned break times and attendance. Accept assigned duties in a cooperative manner; and perform all other related duties as assigned. Participate in on call rotation for after-hours participant needs. Be flexible in schedule of hours worked. May require use of personal vehicle.

Business Hours: Monday – Friday / 8:00 a.m. to 5:00 p.m.
Requirements:
- Graduate of an accredited Nursing program
- Bachelors of Science in Nursing, preferred
- Current California Registered Nurses license
- Minimum of three (3) years of health care experience with emphasis in geriatrics.
- Minimum of one (1) year of documented experience working with a frail or elderly population.
- CPR certification
