Overview:
Summary/Objective
Under the Supervision of the Corporate Compliance Officer, the Health Care Support Coordinator is responsible for the overall oversight and support of the healthcare needs of the people we support within the FREE network of services. The goal will be to proactively focus on programs that require additional support through staff observation, staff training, medical records and data review, documentation and follow up. The Health Care Support Coordinator will work in conjunction with the Nursing Administrators, Registered Nurses and all Operations Leads to ensure the delivery of the highest quality of integrated health and continuity of care to the people supported in the organization.
Responsibilities:
Essential Job Functions
  • Review and evaluate data collection on individuals within assigned caseload and supporting programs including, but not limited to, lab results, diagnostic testing, BM data.
  • Observe and evaluate staff performance related to individual’s medical care and environment in a safe manner.
  • Review and evaluate the development of individual’s plans and assist in identifying areas of need.
  • Assist with training of Direct Support Professionals for assigned programs as well as supporting programs. Providing additional training as indicated.
  • Assist in observation and evaluation of nursing care administered by Direct Support Professionals.
  • Review and document pertinent observations and interventions as required by policy and regulation: including, but not limited to, AMAP protocols, BM protocols, Feeding protocols, diabetic protocols.
  • Assist in maintaining effective communication with Inter-Disciplinary Team members.
  • Communicate information regarding assigned programs and supporting programs to the Corporate Compliance Officer
  • Review the use of agency systems for oversite of medical appointments, medication administration, staff training when necessary and/or requested by Nursing Administration
  • Ensure compliance with regulatory standards.
  • Work in partnership with nursing and operations team to design and launch trainings that will promote the skills of all team members working with the individuals supported.
  • Ensure appropriate protective and preventative measures are followed by staff and individuals in the control of communicable and infectious diseases along with the identification, assessment, and reporting of the same.
  • Attend Inter-Disciplinary Team Meetings, Project Management Meetings; Integrated Care Committee to provide guidance and support.
Duties and Responsibilities
  • Adhere to a flexible schedule that reflects, and is based on, the needs of the programs supported. If warranted, change in program location and oversight may occur.
  • Work in conjunction with the Corporate Compliance Officer and Nursing Administration to provide additional support to identified programs. Support will be provided based on area of need: staff training, medical record review, data review nursing vacancies, etc.
  • Ensure adequate support is provided to the nursing, residential staff and programs we provide service to.
  • Always communicate in a respectful manner with all people including the people we support, supervisors, other valued team members, and members of the community.
  • Complete all required trainings and stay in compliance.
  • Maintain the confidentiality of any information relating to individuals and employees of the organization.
  • Other duties as assigned.
Qualifications:
Education, Work Experience, and Competencies
  • Valid Current NYS RN License.
  • Minimum 1 year experience as a Registered Nurse, preferably with the differently abled.
  • Experience working with the differently-abled population preferred.
  • Knowledge of OPWDD and OMH state and federal regulations preferred.
  • Must be efficient, well organized, and have excellent communication skills.
  • Willingness and ability to travel between program sites.
  • Ability to manage multiple priorities and ability to be flexible to meet the needs of a diverse population.
  • Valid NYS Driver’s License and satisfy criteria to be a driver for FREE.
Physical Demands
  • Employee is required to sit, walk and stand for prolonged period of time (at least 8 hours)
  • Employee is required to effectively communicate both verbally and in writing
  • Employee is required to operate standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines
  • Employee is required to reach with hands and arms
  • Employee is required to bend, stoop, kneel, crouch or stand on a stool as needed
  • Employee is required to be able to lift 50 pounds at a time
