Registered Nurse (RN) Home Health Case Manager: $70,000 - $75,000

Together Homecare of Indianapolis is looking for a passionate and driven Registered Nurse Clinical Supervisor to join our team! We are looking for teammates who share our vision of helping as many people as possible in our community thrive in the comfort and safety of their homes. In this role, you will plan and supervise the care for your own patient census, working with our clinical and operational departments to ensure safe, effective care is provided to our patients.

Requirements:
- Registered nurse with an active Indiana license or compact license
- 1+ year of nursing experience preferred, but not required. NEW GRADS WELCOME!
- Reliable transportation with an active Driver’s License
Expectations:
- Manage all elements of the course of homecare, including care planning, goal evaluation, coordination of care, and care supervision/evaluation.
- Ensure timely and accurate communication and documentation, both in the office and in the field.
- Provide education to external team members, patients, families, and referral sources.
- Participate in care coordination and transition management activities across the healthcare continuum.
- Ensure effective communication to patients, family members, and physicians.
- Participate in staff meetings, in-services, and agency performance improvement initiatives.
