Company Summary: Care Advantage is a home care company that specializes in one-on-one quality personal care and companionship in the home, hospital, nursing home or rehabilitation center. We tailor make care plans that work for our clients.
A DAY IN THE LIFE OF A HOME CARE RN:
- Makes initial contact with the patients and explains services offered
- Completes start of care assessments and documentation to onboard new patients
- Designs an individualized plan of care for each patient
- Instructs assigned care team members on the needs and care plan details
- Reports progress of care to involved parties
- Follows up and maintains relationships with clients, always advocating on their behalf
- Establishes relationships with community partners and referral sources
- Keeps Clinical Leadership and Office Administrator abreast of patient condition and/or medical changes
- Reviews caregiver documentation
- Takes part in interdisciplinary team meetings to ensure needs of all patients are met
- All other duties as assigned by Administrator and job description
Requirements:
- Must possess an active Commonwealth of Virginia Registered Nurse license
- Minimum of 1 year in a Nursing role- LPN to RN acceptable
- Minimum of Associates Degree in Nursing
- Comfortable working collaboratively in cross-functional teams
- Ability to communicate effectively, both verbally and in writing, with all levels of an organization
- Excellent presentation skills
- Adequate transportation for home visits with mileage reimbursement
- Computer/iPad Technology skills for documentation purposes
