JOB SUMMARY: The Rehabilitation Specialist assists in the planning, organization, and implementation of therapeutic programs/interventions (sports, group games, art, life skills) appropriate for residents’ treatment. The position is additionally responsible for assisting the mental health team of the youth in all areas of the therapeutic program. responsible for providing therapeutic behavioral interventions and replacement skill-building strategies for clients, assisting as a mental health worker, and intensive home-based services. Rehab Specialist partner with child and therapeutic team to achieve and maintain the goals as defined in the treatment plan and are responsible for daily progress notes and other required documentation.

MINIMUM QUALIFICATIONS:

Education:

1) Bachelor's degree in psychology, social work, childcare counseling, education or development from an accredited college or university is required.

2) Four (4) years direct service experience with severely emotionally disturbed residents in an inpatient or outpatient health care setting (internship and/or practicum time may be substituted).

Experience:
One (1) year direct service experience treating emotionally disturbed children.

License or Certification:
Valid California driver's license.

Specialized Skills:
Ability to interact and communicate both verbally and in writing with seriously emotionally disturbed children, other staff, physicians and the public.
Demonstrated understanding of psychiatric illness and its treatment.
Demonstrated understanding and knowledge of community services and county agencies.

Physical Requirements:
Ability to visually and audibly assess resident's behavior and needs.
Ability to sit at a desk and communicate over the telephone for extended periods of time each day, up to 1 hour at a time.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

1) Ability to interact and communicate both verbally and in writing with seriously emotionally disturbed residents, other staff, physicians, and the public.

2) Demonstrated understanding of psychiatric illness and its treatment.
3) Demonstrated ability to assess the mental status and psychosocial needs of severely emotionally disturbed residents visually and audibly.
4) Demonstrated understanding and knowledge of community services and county agencies.
5) Under supervision of the Head of Service, assists in performing evaluations and periodic re-evaluations of residents describing the level of psychosocial impairment, physical health problems, self-care potential, support network availability, adequacy of living arrangements, financial status, employment status and potential training or educational needs.

6) Visually reviews medical, psychosocial, school, and other records.
7) Meets with residents, families, treatment staff, placement staff, consultants, and other agency personnel to evaluate needs and services for the resident.
8) Aids in the development of the written individual Mental Health Treatment Plan and Crisis or Safety Plans as needed, identifying, and linking the resident and family’s needs and strengths with the identified goals negotiated with the resident and family.

9) Intervenes by telephone or in person with the resident in an emergency crisis to provide support and assistance in problem resolution.

10) Provides interventions for the resident and family to improve resident’s behavioral outcomes as indicated on the
Mental Health Treatment Plan and make recommendations on funding allocations to Head of Service.

11) Monitors, supports, and assists the resident in developing or maintaining skills needed to achieve the Mental
Health Treatment Plan, including medical, dental and psychiatric services, guidance in money management, and the use of educational, socialization, rehabilitation and other social services.

12) Identifies, assesses, and mobilizes resources to meet the resident's needs including consultation and intervention with social security, welfare, and health departments and other community agencies and resources, as necessary.

13) Interacts with residents and staff as needed in the coordination of services.
14) Interacts on behalf of the resident with family, friends, legal guardian, and significant others to initiate and maintain supportive networks. Acts as liaison with the resident’s Child and Family Team (CFT) to attend meetings, participate and advocate for the resident and family, and ensure action items are followed.

MENTAL DEMANDS the incumbent in this position must be able to accommodate to any/all of the following: constant distractions, interruptions; watching or waiting (e.g. at night); confinement to inflexible work schedules and/or procedures; uncontrollable changes in priorities/work schedules; long periods of exposure to complaints/problems and inappropriate behavior and language of others; long periods of constant contact with others where no privacy is possible; and depending upon assignment, be able to work variable schedules.
