Job description
We are seeking a passionate, highly-skilled Family Nurse Practitioner or Physician Assistant for School-Based and Outpatient Primary Care in the White Mountains of New Hampshire. This is a permanent position.
Responsibilities
- Provider must have minimum of 3-5 years in practice in outpatient primary care
- 3 days providing care in local schools to school-aged children, 2 days per week in Groveton clinic providing care to adults and children.
- Grant-funding through the State of NH
School-based grant services to increase access to health services and behavioral care for children in our catchment area. Will include care coordination with the PCP.
- Preventative health exams
- Complete physician examinations
- Acute care appointments for new or existing health problems (in person or via-telehealth)
- Education on health-related topics
- BMI assessments, nutrition education and physical activity counseling
- Immunizations
- Health education specific to diabetes and asthma
- Medication management
Primary Care:
- 20-24 patients per day
- EMR is Meditech Expanse, Dragon dictation
- Supported by an RN Team leader and designated MA
