JOB SUMMARY:* This position is responsible for supporting seniors in living independently in their homes as long as is safely possible. We are a small team with a wealth of experience who provide client centered, strengths-based support to seniors. We provide resources, service access and coordination, advocacy, and support to seniors through Oregon Project Independence, Options Counseling and Family Caregiver programs.
We work in SE Portland and provide visits to clients in their homes. We believe equity and inclusion are actions as well as values. We are looking for someone who is passionate about working with seniors in our community, works independently but also connects with our team. The right candidate for this position must uphold the mission, vision, and values of Impact NW.

JOB RESPONSIBILITIES:*
- Provides case management services including development of a strength-based client relationship, comprehensive needs and strengths assessment, client referral and advocacy, service coordination, development of outcome-based support plans and follow-up within all timelines as directed by contracts, funders, and Program Director.
- Assesses clients for available programs, completes all required forms, meets deadlines for client services, attends trainings and applies training information to their work practices.
- Encourages and facilitates client participation in offered resources and programs.
- Facilitate service coordination with internal and external services or resources in order to provide comprehensive support to the client.
- Shares responsibility for center coverage including crisis intervention and reception.
- Completes accurate, legible, and timely documentation of services delivered in accordance with Impact NW's management information system.
- Meets client service standards set forth by Program Director regarding quantity, type and quality of services to be delivered to clients.
- Conducts home visits with clients and is able to get to these appointments efficiently.
- Support the client to hire and manage a home care worker and process payments.
- Effectively works with individuals of diverse backgrounds.
- Performs other duties as assigned.

REQUIREMENTS AND QUALIFICATIONS:*
Any combination of experience and training that would likely meet the following requirements
- Bachelors degree or equivalent work experience in social work or related field.
- Minimum of two years’ experience in human services, preferably working with seniors.
- Ability to effectively work within a strength-based, integrated family support service delivery model.
- Ability and willingness to work as a team member, use positive and direct communication with team members, and support Impact NW's mission and goals.
- Ability to communicate effectively, both orally and in writing.
- Sensitivity and ability to relate to people from diverse lifestyles and cultures.
- Possess good organizational skills and ability to prioritize multiple and varied tasks.
- Demonstrated computer skills using multiple databases, Microsoft Office Programs, the Internet and the Cloud
