Nye Health Services, a family-owned company that has been recognized nationally for our care, service, and culture is seeking a Full-time Social Services Coordinator for our Gateway Vista campus located in Lincoln, NE. In this role you will:
- Assist with the resident/tenant admission process ensure appropriate transition and plan of care is in place.
- Interview and evaluate residents/tenants to ensure they are appropriate for placement at the campus including the PASARR screening.
Coordinate with campus leaders, staff and care team to help meet the variety of physical, emotional, intellectual, cognitive, spiritual, and psychosocial needs so that they may maintain their highest practical level of well-being and functioning.

Act as a liaison between and confer with the resident and their family/representative to best meet the variety of needs and capabilities of the resident.

Monitor, assess, and address residents for Social Services related concern such as changes in physical, emotional or cognitive condition or status; likes/dislikes; financial concerns/resources (including but not limited to Medicare, Medicaid, insurance); behavioral issues; room changes; leisure pursuits; transportation needs; Advance Directives; Resident/Tenant rights issues; personal needs/shopping; referral needs/resources to appropriate agencies when campus does not provide the services to meet the resident/tenant needs; and other medically related social work needs.

Coordinate discharge planning, development, and implementation to assist with a successful transition.
Qualified candidates will have:
- Minimum of a Bachelor’s degree in Social Work, Psychology, Sociology, Counseling, or related field, and/or NHCA 36-hour Social Services Designee course.
- One-year related experience and/or training in Long Term, Acute Care, and Social Work in management preferred.
- Excellent communication skills and a customer services orientation.
- Will maintain required credits in accordance with licensure requirements as needed.
- Ability to work a required weekend rotation in support of the admission and discharge process.
