The Social Services Coordinator will facilitate support of the resident through the admission, transfer and discharge process and daily with living issues, through social guidance and regulation.

Responsibilities:

  • Coordinate entire admission process for Medicare residents.
  • Facilitate smooth transition for customers through effective and proactive communication.
  • Assist client/family to complete application for admission and all other admission paperwork in accordance with state and federal laws.
  • Complete social history.
  • Educate resident and family of Medicare guidelines.
  • Outline and provide discharge plan (home health services, DME, caregivers, community resources).
  • Interact with Interdisciplinary Team and involves family in care plans.
  • Involve families in resident care by timely notification of care plan meetings and provide appropriate follow up when families are unable to attend.
  • Provide crisis management services when indicated.

Requirements:

  • Bachelor’s degree from an accredited college or university; related experience and/or training; or equivalent combination of education and experience.
