Social Work Case Manager Position
Utah Case Management is looking to add Social Work Case Managers to our team to coordinate services for clients through Utah Medicaid's New Choices Waiver program. The position can be part-time based on your needs, and we offer the benefit of a flexible work schedule allowing you to govern your own hours and schedule. We are looking for Social Work Case Managers in the Salt Lake County area. See below for responsibilities and qualifications:
Responsibilities:
- Managing clients' services: making sure clients are getting the services they need, coordinating services, etc.
- Documentation: completing, updating, submitting, and compiling necessary documentation for clients.
- Regularly meeting and communicating with clients (virtually and in-person)
- Communicating with other Case Managers to coordinate clients' services
Qualifications
- Utah Resident
- Current SSW license, CSW license, or RN License in the State of Utah
- Good at maintaining relationships with clients
- Detail oriented
- Independent
- Must have daytime availability for training and making phone calls

Utah Case Management is a case management agency that works with individuals on Utah Medicaid's New Choices Waiver. Our mission is to enhance the lives of individuals on the New Choices Waiver Program by providing services and supports that enable our clients to thrive and participate in their communities. Together with our team of nurses and social workers, we provide clients with assistance in locating, accessing, coordinating, and monitoring needed services that are available to them through this program.
Utah Case Management employs social workers and nurses throughout Utah. We highly value flexibility therefore our Case Managers are given the benefit of a flexible work schedule allowing them to govern their own schedule and hours.
Job Type: Part-time
Pay: $15.00 - $26.00 per hour
