Job Summary:
The Department of Social Work at SUNY Downstate Health Sciences University is seeking a full-time Social Worker Assistant.
The Social Worker Assistants are an integral part of the hospital multidisciplinary team. The Social Worker assistants are responsible for the coordination and delivery of care services to patients and their families. They educate patients, family and staff on diagnosis, treatment programs and available resources. The Social Worker Assistants conduct psychosocial, emotional, and socioeconomic screenings and assist in implementing social workers ’care plans. They work collaboratively with other members of the patients' care team to assure high quality of care. They provide emotional support, resource information, assist in legal reporting when needed, long-term care planning, etc. Additional responsibilities include:
- Assist the social worker in overall coordination for mental health and social services for patients of SUNY Downstate Primary Care Ambulatory Care Department.
- Assist the medical team with assessing the psychosocial needs of patients and refer them to appropriate social services and behavioral health services, internal or external, as needed.
- Engage family members/caregivers to discuss patient status and provide supportive guidance where indicated.
- Assist patients with obtaining durable medical equipment, medications, medical supplies and all other services that will improve patient home environment and self-management abilities.
- Connect and reconnect patients to long term care services such as Managed Long-term Care Plan (MLTC), FIDA, MAP, HARP programs.
- Collaborate in a team-based care model with all members of the patient care team including PCPs, nursing, behavioral health and support staff.
- Receive warm handoffs from patient care team.
- Provide feedback to the patient care team on a regular basis in regards to the status of the patient and all interventions provided.
- Conduct a face to face and phone contacts with patients.
- Perform all other related tasks as needed and as business needs warrant.
Required Qualifications:
- Bachelor’s Degree in Human Services, Social Work or other related field.
- Knowledge of Federal, State and TJC regulations.
- Knowledge of Medicare, Medicaid and Managed Care Standards, requirements, benefits and benefits administration.
- Knowledge of state and federal programs that provide financial support to underserved population.
- Knowledge of community resources.
- Proficiency in use of PC, Microsoft Office Suite and clinical Applications.
- Ability to work with diverse patient population with knowledge and sensitivity to cultural and spiritual influences impacting on patient care social work assessment and counseling skills, grief and bereavement counseling, alcohol and drug intervention, and support for victims of abuse, neglect and violence.
- Possess a positive attitude, motivation and good interpersonal skills, excellent verbal and written communication skills.
- Good problem resolution skills, ability to work in a multidisciplinary-team approach and ability to work independently with minimal supervision.
