Position Requirements

· Bachelor’s Degree: Social work or pursuing degree for same

· Previous experience in caring for chronic disease patients required

· Possesses strong critical thinking skills

· Can pay attention to detail and accuracy of documentation in records

· Demonstrates evidence of effective organization, leadership communication, education collaboration, and counseling skills

· Previous Care Coordination, Case Management, Home Health or Behavior Health experience preferred

· Previous experience with mobilizing community resources, navigating patients through the healthcare continuum, and working with disparate populations preferred

· Ability to speak Spanish preferred

· Ability to identify and implement appropriate patient communication strategies

· Previous experience with health IT systems and data reports preferred

· The demonstrated ability to work constructively with all disciplines related to caring for patients within the community

· Familiarity with community resources

Responsibilities

· Meet with patients/families and identify and address various challenges that patient may face from social/environmental/psychological/financial/legal/other perspective that limit ability of patient to fully participate in their care plan

· Coordinate social support programs/transportation/ housing/other to address patient challenges

· Provide educational resources for patient to better understand their treatment plans

· Monitor patient progress and evolve plan as needed

· Work closely with the Care Manager to address patient challenges

Job Type: Full-time

Pay: From $50,000.00 per year
