Perry Lutheran Homes is looking for a self-motivated social worker to provide direct and indirect services to residents of Perry Lutheran Homes – Willis campus (long-term and memory care) and Spring Valley campus (assisted living). Services will include but not be limited to assessing, identifying and assisting in meeting the social, emotional, physical, and behavioral needs of our amazing residents, tenants and their families.

Job Responsibilities:
- Plans, develops, organizes and provides social services programs that meet individual resident’s psychosocial needs.
- Interviews resident/family members, documents the social history and evaluates the social, emotional, and personal needs of the resident upon admission to facility. Actively involves each resident and family members in planning objectives and develops social service programs tailored to the goals, personal needs and requests of the resident.
- Develops care plans and keeps them up to date as per RAI manual.
- Facilitation of and participation in care conferences with residents, tenants and families along with other members of the interdisciplinary team.
- Provide opportunities for residents/tenants or families who are struggling with issues to express these struggles in a safe, non-judgmental forum. Assist with advocacy and problem-solving strategies.
- Administers BIM’s & PHQ9 assessments on residents. Communicates as appropriate to Director of Nursing and/or physician.
- Participates in pre-admission screenings, assessments, discharge planning and recommendations of residents. Coordinates changes affecting the residents with staff members, residents and families.
- Consults with Administrator, department heads, staff, community agencies, medical professional, etc. to meet the needs and goals of the resident to ensure a positive mental, social and home-like environment for the resident
- Co-facilitate resident council meetings.
- Participate in committees in house, especially related to behavioral concerns.
- Participate in relevant community activities that promote the mission of PLH.
- Prepare and present educational programs on topics related to long-term care, aging, family issues related to aging, dementia and others when requested.
- Institute “guardian angel” program within facility to promote staff/resident/tenant relationships.
- Facilitate community Alzheimer’s Support Group.
- Keeps abreast of current federal and state regulations, as well as professional standards, and makes recommendations to the Administrator on improving resident directed care initiatives that meet regulatory requirements.
- Informs staff of pertinent psychosocial needs of individuals
- Assists with complaints from residents and/or family members and recommends solutions
- Provides information and resources to residents and family members on Medicare/Medicaid or other financial assistance programs, and assists with other personal financial/business matters as appropriate.
- Team member of MDS process with assessment of mental, emotional and behavioral items on MDS screenings.
- Assists with Advance Directives (living will, durable power of attorney for health care decisions) for residents.
- Participates with and supports Marketing Director and Administrator in community relations. Maintains educational and professional contacts. Participates in Social Worker associations as appropriate.
- Meets with Marketing Director routinely to provide input on upcoming admissions and transfers. May conduct tours and handle phone inquiries, as requested.
- Completion of documentation utilizing electronic health records for admission assessment, quarterly assessment, annual assessment, significant change assessment as well as documentation of resident and family contacts that impact their care.
- Maintains all records, care plans, forms, and reports (including regulatory) on the status of the residents and submits to the administrator as requested
- Maintains confidentiality of resident information and privacy rights. Disclosure on absolute “need to know” basis.
- Facilitates in-service meetings on resident right’s, dependent adult abuse, and other educational offerings as requested.
- Provide consultation to or attend various committees including but not limited to Infection Control, Policy, Advisory, Budget, QAPI, Safety, Resident Council, etc. as directed by the Administrator.
- Will actively participate in QAPI program, taking a proactive approach to quality care and services on a continuous basis with engagement of residents, family members and staff.
- Advocates for the residents
- Promotes and supports all safety programs to provide the utmost safety for the residents and employees.
- Performs other duties as directed.
Requirements:
- Bachelor’s Degree in Human Services preferred. Consideration will be given to individuals with Associate’s Degree and relevant experience in long-term care.
- Knowledge of long-term care systems, funding and level of care; knowledge of geriatric stage of life and issues that relate to social, emotional, and behavioral concerns of that stage; basic knowledge of commonly used geriatric psychotropic medications and effects; knowledge of dementia stages and effects on individuals and families.
- Excellent verbal and written communication skills.
- Effective mediation, problem-solving, prioritizing, and implementation skills.
- Ability to work well in team setting with diverse backgrounds.
- Desire to work with and serve residents and tenants in a way that is compassionate, personalized, and respectful
