Job description
Serves unsheltered homeless persons in the community, engaging them by either referral or identification by other agencies, such as law enforcement or state services.
This position is supervised by Community Missions Executive Director.
Essential Job Functions – Responsibilities
1. Engage individuals experiencing homelessness on the streets, encampments, and other public access areas within the St Joseph CoC.
2. Focus outreach on the most vulnerable individuals including the chronically homeless and other vulnerable populations (veterans, families with children, women). Provides access and referrals to appropriate services
3. Use “best practice” methods, including motivation interviewing, trauma informed care, and harm reduction, to engage homeless persons to encourage them to accept housing placement.
4. Assist clients with immediate needs including food, water, and emergency shelter.
5. Conduct assessment of needs and assist clients in obtaining appropriate services. Network with community agencies for the purpose of obtaining information and coordinating service referrals.
6. Provides transportation for clients through bus passes or actual transport for obtaining identification materials and employment opportunities
7. Encourages housing opportunities including, when appropriate, completing steps through the Coordinated Entry System.
8. Maintains appropriate and accurate records of clients including documentation in HMIS with appropriate program enrollments and referrals.
9. Verification of homelessness for agency documentation and housing program enrollment.
10. Attend meetings as required including Coordinated Entry Case Conferencing, CoC, and others designated by supervisor.
11. Submit reports to accounting for grant billing purposes.
Job Specifications
1. Education: Bachelor level degree in Social Work or related field, OR two years working with homeless population in some capacity
2. Licensure: N/A
3. Experience: Experience in working with low-income, mentally ill, homeless, substance abuse or at-risk populations. Experience in de-escalation.
4. Essential Technical/Motor Skills: Proficiency in a variety of computer software applications including Microsoft Office Suite, HMIS data, email, and internet applications
5. Interpersonal Skills: Strong communication skills, patience, ability to listen, ability to nurture relationships and relate to diverse populations, ability to initiate new contacts and demonstrated leadership skills.
6. Essential Physical Requirement: Ability to remain in a stationary position for up to 50% of the day. Ability to position self to access upper and lower files, cabinets, and shelves. Manual dexterity to enter data into the computer. Ability to ascend and descend stairs. Ability to negotiate spaces within client living areas.
7. Essential Mental Abilities: Ability to complete projects; ability to develop short- and long-term goals. Ability to work independently. Ability to complete all necessary paperwork on schedule.
8. Essential Sensory Requirements: Ability to communicate effectively in person and on the telephone, ability to review records for completeness and accuracy
9. Exposure to Hazards: Client contact in places including places not meant for human habitation, encampments, bridges, and other locations unsheltered homeless individuals stay.
10. Other: Provide own reliable transportation. Valid Missouri Driver’s License and vehicle insurance required, ability to pass a background screening by Family Care Safety Registry.

Pay: $30,000.00 - $33,000.00 per year
