RESPONSIBILITIES
- Conduct initial contact with clients via telephone as well as through zoom
- Ability to complete appointments on Zoom and use Docusign to obtain signatures
- Complete Medicaid eligibility process, initial visit with clients and caregivers
- Ensure consent and orientation documents are complete
- Complete progress notes on a daily basis
- Complete Authorization request to Medicaid providers
- Referrals completed to needed initial services (mental health assessment)
- Assign clients to Targeted Case Managers
QUALIFICATIONS
Candidates must have a bachelor’s degree from an accredited university or college with a major in counseling, social work, psychology, criminal justice, nursing, rehabilitation, special education, health education, or a related human services field (a related human services field is one in which major course work includes the study of human behavior and development) and have a minimum of one year of full time or equivalent experience working with children with serious emotional disturbances and adults experiencing serious mental illness;
OR have a bachelor’s degree from an accredited university or college and three years full time or equivalent experience working with children with serious emotional disturbances. Candidates must have some knowledge of the Medicaid manual and be willing to be certified by Medicaid within 90 days of hire.
Other requirements:
- Must have knowledge and must comply with state and federal statutes which affect the target population
- Excellent verbal and written skills a must
- Must have a valid FL Driver’s License and excellent driving record
- Must successfully complete all criminal background screenings annually
- CBHCM certification required when eligible
