Job Description

Saint Joseph's Medical Center is seeking a full time Team Leader to supervise a team of Care Coordinators in providing person-centered assessment, care planning and care coordination in according to program guidelines and procedures.
Requirements:

Master's Degree in Human Services

License preferred

Valid Driver's License Required and must own vehicle.

This position is location in our Yonkers Campus- Saint Joseph's Medical Center
