Planned Parenthood Keystone (PPKey) serves 37 counties in Pennsylvania, home to over half of Pennsylvania residents. We have administrative offices in the Lehigh Valley, Harrisburg, Warminster, and York. We operate 9 medical centers that serve over 22,000 patients annually and offer comprehensive sexuality education that reaches thousands of young adults.

An abortion patient navigator is a community health professional that focuses on the abortion patient’s needs to access care on both an individual basis, and at a systems level. The patient navigator helps guide the patient through the abortion care system and works to overcome obstacles faced by the patient receiving the care and services they require, while working to improve the systems and processes that shape access at the clinical and ecosystem levels

ESSENTIAL DUTIES AND RESPONSIBILITIES:

  • Complete training for abortion counseling to provide education as needed
  • Organizing schedules and managing appointments for patients to ensure they receive services in a timely manner and at a location most convenient for them based on the patient’s clinical care site preferences, or offer Direct to Patient MAB if applicable.
  • Contacts patient prior to initial visit: Confirms patient plan to keep scheduled appointment, ensures compliance with state mandated video, answers questions about financial services and travel assistance, confirms valid ID or birth certificate (for minors) advises patients that they cannot bring support person to appointment, provides support for follow-up care.
  • Facilitating communication between the patient, support person(s) and/or organization(s), and healthcare providers to ensure patient satisfaction and quality of care.
  • Informing the patient on financial aspects of care and linking the patient to financial resources as needed (i.e., Justice Fund, Abortion Liberation Fund-PA, Fund For Choice)
  • Working with the patient to identify logistical needs to access care and linking the patient to appropriate resources as needed.
  • Working with the patient to identify other support services as needed, and providing appropriate linkages
  • Accommodating interpretive language services when needed
  • Facilitating pre and post-abortion visits and ensuring continuity of care and linkage to community resources
  • Collecting, tracking, and reporting required program data
  • Identifying and addressing existing gaps and opportunities within existing patient navigation processes, including screening processes, to ensure highest-need patients are being identified, routed, and connected to navigational support
  • Working with external stakeholders and organizations to build and strengthen relationships, systems, and processes that support access to abortion care
  • Working with external vendors to facilitate access to key services, including but not limited to, transportation and lodging
  • Working closely with affiliate and contact center staff, including but not limited to social workers, Justice Fund managers and counselors, contact center staff, health center managers, and others to create efficiencies and improve workflows for increased abortion access and improved patient experience
  • Developing, managing, and advancing program goals and objectives for new and/or improved processes and systems that increase access to equitable abortion care

Serve as the main point of contact for:

  • Health Center management, clinicians, and support staff to develop systems and establish procedures to support the success of abortion services including, but not limited to patient communication, workflow, optimal utilization of EHR, patient experience, auxiliary services, and monthly case review with the clinical team.
  • Call health center if available appointments do not work for patient, or if demand increases (i.e. need for waitlist, etc), or ability to overbook templates.
  • Oversees abortion services phone line; ensure return of all phone calls from patients and non-patients regarding abortion services, and appointments. Task any clinical needs directly to clinicians. Task any education requests to the Director of Education. Goal: all patient calls will be returned within 24-48 business hours.
  • Serve as main point of contact for outside resources related to abortion services (i.e., Justice Funds, Abortion Liberation Fund-PA, Fund for Choice)
  • Supports efforts of the agency’s diversity program to provide services that are culturally and linguistically competent; takes initiative to develop own awareness and knowledge of diversity matters.
  • Stay current with PPKeystone’s Abortion Services as well as general trends in abortion care by attending appropriate trainings, conferences, reading relevant journals, and otherwise ensure ongoing education in the field of abortion care.

An effective patient navigator should have a demonstrated ability to quickly and effectively synthesize patient needs and liaise with providers and local resources to create a unique patient care plan. To that end, patient navigators should possess the following characteristics:

  • Excellent project management skills

  • Compensation for this position starts at $19.25 per hour and goes up from there based on experience
