The Purpose:
To deliver resident specific care in order to provide services to maintain and/or improve skin integrity. Collaborate with physician and licensed nurses to develop and modify the treatment regimen to meet the physical and psychosocial needs of the resident, in accordance with established medical practices and the requirements of state and federal regulations and the guidelines and goals of this community
Responsibilities:
  • Provide clinical supervision to nursing assistants. 6. Administer all care and treatment according to the physician’s order and accurately document all in the clinical record.
  • Document resident care provided and resident’s response or lack of response to care and treatment provided.
  • Assess and report changes in resident’s condition including development of decubitus to physician, Charge Nurse, Nursing Supervisor, Director of Nursing Service and take follow-up action as necessary.
  • Instruct licensed nurses and nursing assistants in proper, preventative safety measures and use of adaptive equipment to meet residents’ needs.
  • Assess each resident’s skin condition weekly, and as necessary, including stage, size (measurements), sites, depth, color, drainage and odor of each condition
  • Other duties as assigned
Qualifications:
Texas: Must have a current and valid Texas RN nursing license or a Texas LVN nursing license or a current and valid RN/LVN/LPN license from a Compact Party State. The RN or LVN may continue to practice under the former home state license and multistate licensure privilege during the processing of the nurse’s licensure application in the new home state for a period not to exceed thirty days
