The Family Support Coordinator will work one-on-one with WIC participants who need enhanced support and referrals for finding appropriate local resources to help with issues and concerns such as housing, jobs, childcare, healthcare, food insecurity, or mental health.

Qualifications:

• 
  Bachelor’s degree with a major in social work, public health, education, psychology, counseling, or human services. Experience may be considered in lieu of degree for a maximum of two years of the required experience.
• Previous experience in health/ human services case management and/ or the field of substance abuse, smoking cessation, domestic violence, housing, or counseling.
• Experience in community partnerships.
• Ability to work effectively with diverse cultural groups and socio-economic backgrounds.
• Bilingual preferred.
• Computer skills required – Microsoft Office, Excel.

Responsibilities:

• 
  Assist WIC participants referred to them by nutrition staff to identify and navigate referrals for support services.
• Work collaboratively with other area resources in empowering clients to access services leading to self-sufficiency.
• Complete intake and assessment process on all potential clients utilizing appropriate forms to track and report referrals.
• Ensure ongoing follow-up to evaluate the receipt of services, as needed.
• Coordinate with WIC Program Director and Senior Nutritionist to develop a triage system to assist participants in addressing issues or concerns outside of nutritional needs.
• Coordinate with the WIC Community Coordinator to identify community referral resources.
• Attend Family Support Quarterly meetings, trainings, and in-services.
• Maintain WIC confidentiality policies.
• Provide ongoing support to WIC sites.

License Requirement

Travel is required. Must have a current and valid Massachusetts Class D Motor Vehicle Operator’s license or equivalent from another state.
