Shift: 8am-4:30pm

Horsham Center for Jewish Life, in the Phiadelphia suburb of North Wales, PA, is searching for an experienced and detail-oriented Full Time Wound Care Coordinator to support the team Mondays-Fridays. Responsibilities

  • The primary purpose of the job position is to develop, implement, and supervise day-to-day wound care program of the facility in accordance with established nursing standards, the policies, procedures, and practices of this facility and the requirements of current federal, state, and local standard governing the facility
  • Make written and oral report/recommendations to the Director of Nursing as necessary/required, concerning the operation of the nursing services department as it specifically relates to wound care coordination
  • Participate in the development, maintenance, an implementation of the facility’s quality assurance program for the nursing service department
  • Attends and participates in weekly wound care meetings
  • Responsible for accurate and timely documentation of wounds in the nurse’s notes at least weekly
  • Responsible for all treatments required by licensed nurse on your shift and ensuring the wound care treatments are carried out at other times
  • Evaluate high risk residents for pressure reduction intervention methods
  • Conduct body assessments & documentation on all new admission per policy

How to be a successful candidate:

  • Have at least 1 year of experience working in a Long-Term Care or Rehabilitative facility
  • Have excellent oral and written communication skills
  • Embrace our person-centered care approach to meet each resident's needs and preferences
  • Must be knowledgeable of nursing and medical practices and procedure and laws, regulations and guidelines governing long-term care.

Qualifications

Must have an active RN license in the state of PA.

Previous long-term care experience is preferred. Wound care certification desirable
