JOB SUMMARY: The Youth Coordinator provides support services to middle school students aimed at reducing truancy. The position carries an assigned caseload to provide case management services to address issues and challenges that the youth is experiencing in getting to school daily and promptly. The Youth Coordinator oversees Youth Service Providers contracted by the Agency to provide youth workshops within the school.

ESSENTIAL DUTIES and RESPONSIBILITIES:

  • Provide support services to middle school students aimed at reducing truancy.
  • Engage and assist middle school students with identifying areas of difficulties contributing to issues related to truancy.
  • Determine school-based activities that would best serve to reconnect the student to their academics and reduce absenteeism.
  • Write and review individual service plans (ISPs) to address the needs of the student.
  • Lead group sessions at least once or twice a week that will provide a safe place for students to address and problem-solve truancy issues, academics and other concerns.
  • Work closely with the Community Advocate in completing one face-to-face visit with each student’s family.
  • Collaborate with Community Advocate should family support services need to be provided to the family.
  • Make telephone contact with student and student’s family as needed.
  • Provide on-call after hour support for emergencies. Follow up with student/family the next day.
  • Provide assistance to the student and student’s family as needed.
  • Provide referrals to student and student’s family as needs are identified.
  • Write progress notes for each contact with student and student’s family.
  • Complete all required paperwork on each student per department guidelines.
  • Attend all agency and contract trainings and meetings as required.
  • Perform other job-related duties as assigned.

EDUCATION and EXPERIENCE:

  • High School Diploma or equivalent GED.
  • Two years of experience in the human services field working with youth and families.

SKILLS and COMPETENCIES:

  • Communication skills, verbal and written.
  • Interpersonal skills.
