Our non-exempt (hourly), ACT/Addictions Nurse LPN helps to support our mission and team so we can work in the most efficient and effective way. This position will work during the core business hours, with no weekend work. You will need to travel to other Adult & Child Health locations and into the community as needed.

Our Assertive Community Treatment (ACT) Team provides services necessary to house individuals who are homeless in Indianapolis. The Addictions team provides monitoring of medication compliance, providing education and training about medications, wellness, monitoring medication side effects and providing other nursing assessments.

The ACT/Addictions Nurse works as part of this multidisciplinary team to provide medical education and healthcare support to Program participants. Nursing services are provided in the clinic and out in the community.

What you will do:

  • Gather pertinent health data and assesses patient's general condition
  • Provide basic patient care as ordered
  • Document care provided or procedures performed accurately as well as assessments, interventions, patient/family responses, medication dispensed/prescribed, and test results in medical records
  • Educates patient/family by way of educational materials
  • Coordinates care of patients with internal and external professionals
  • Has a working knowledge of Ethics and Legal parameters
  • Administer a variety of treatments and medications as directed
  • Medication education and training
  • Collect lab samples as ordered by the provider
  • Adheres to infection control/safety guidelines and confidentiality policies
  • Actively and compassionately interacts with patients to encourage engagement in treatment.

Requirements:


  • Graduate of an accredited School of Nursing and current licensure in the state of Indiana as Licensed Practical Nurse
  • 3 years of LPN experience preferred. 1 year previous behavioral health required.
  • Baccalaureate or Associate Degree in Nursing or Diploma in Nursing
  • CPR certification
  • Knowledge of medical terminology, anatomy, physiology, and pathophysiology
  • Familiarity with health care systems, regulations, policies, and functions
  • Can perform mathematical calculations for drug dosages
  • Ability to use work equipment such as phone, computer, medical charts, stethoscope, echocardiography (EKG) machine, ear-washing instruments, blood pressure cuff, speculums, and other nursing items
  • Communicate effectively in person, by phone and in writing
  • Establish and maintain effective working relationships with coworkers and diverse patient populations
  • Ability to provide care to those with chemical dependency and mental health disorders without judgment, by exercising care and compassion
  • Must hold a valid driver's license with acceptable driving record and provide own, insured, transportation
