What You'll Be Doing:
Care Coordinator is on site at the practice and serves as effective system navigator and point of contact for patients and families, with patients and families having direct access for asking questions and raising concerns. Works in conjunction with the primary care physicians, various specialty physicians, hospitals, and ancillary service providers to maximize the health status of patients in their assigned primary care practices. The Care Coordinator focuses on coordinating care and care plans across all in-patient and out-patient settings, facilitating transitions of care, providing patient health coaching, specifically with high-risk patients, and facilitating compliance with the required quality metrics for all the patients to ensure optimal patient care and communication of pertinent findings to the patient, their care givers and care team. Serves as the liaison between patient, physicians, hospitals, insurance companies, community resources to coordinate the care and resources required to foster positive patient experiences and positive outcomes of care. Advocates for the patient, when necessary, in navigating through the healthcare system. Care Coordinator provides case management services as a resource to the care team for patients with complex support needs. Acts as a referral navigator for the patients so that care is coordinated across the continuum of care.
Work on site with the physician and the care team to manage all patients with special focus on patients with multiple co-morbidities or high risk for readmission to a hospital setting, including development of individualized care plans, maintaining registry and monitoring patient progress.- Care coordinator will be responsible for more that one provider and therefore will require to travel to multiple site
- Is responsible of completing MAWV on site.
- Coordinates continuity of care with patients and families following hospital admission, discharge, and ER visits. Contacts patients within 48 hours of discharge ensuring the medical, psychosocial and functional assessments are completed and appropriate follow up is scheduled.
- Reviews Care Plan with patient and care giver, ensuring patient leaves physician office with a clear understanding of his/her care plan.
- Participates in Patient Centered Medical Home team meetings, population management and quality improvement initiatives, including patient outreach.
- Assumes advocate role on the patient's behalf with the carrier to ensure approval of the necessary supplies/services for the patient in a timely fashion .
