United Helpers, founded in 1898, is seeking a care coordinator with the skills, compassion and commitment to deliver high-quality services to people in northern New York.
Care Coordinators work with individuals who have chronic medical and/or behavioral health issues. They are responsible for linking clients with needed services and supports for successful community living.
Applicants should have strong communication skills and be able to work independently. A Bachelor’s degree in human services, as well as a driver’s license are required.
This is a remote position for an individual who will be working with clients in both St. Lawrence and Jefferson County.
