WHAT YOU WILL DO: This position will provide direct services intensive case management of clients in the Senior Services Program. Case Managers will provide intensive support addressing the needs of the at-risk seniors experiencing food and housing insecurity, medical or behavioral health issues, isolation and/or issues living independently. Working within a comprehensive team of skilled professionals the case managers engage with the Supportive Services team to get the client back to self-sufficiency.
- Provides direct services to clients including assessment, crisis intervention, advocacy, and case management
- Provides guidance and support to help clients identify their own strengths, set achievable goals and follow through consistently so they can become self-sufficient
- Provides evidence-based housing stabilization services and homeless prevention to seniors who are current or at risk of homelessness and have experienced neglect or abuse
- Provides clients with guidance, support and mental health referrals so they can reach their goal of stabilization and self-sufficiency
- Assists clients with conflict resolution issues with family, landlords, etc.
- Meets with clients at their homes and other locations including office, senior centers, faith communities, and other public spaces
- Screens clients for eligibility for all Interfaith programs including housing, basic needs, senior emergency funding assistance and make appropriate warm referrals to internal resources
- Provides outreach to social service organizations, mobile home parks and senior apartment complexes, senior centers, and other organizations that serve seniors
- Links and networks clients with resources within the agency and utilizes other public and private social service agencies as necessary
- Packs and delivers food boxes to clients prepared by the Interfaith Food Pantry
- Administers supportive services per agency and contract guidelines
- Attends and participates in program and agency meetings and trainings
- Creates relationships and opportunities for landlord engagement
- Provides housing navigation services including identifying housing resources and aiding in application process
- Identifies other necessary resources and advocates for clients’ needs
- Maintains accurate and complete records as required by grants and contracts
- Maintains client records of participation, activities, and progress in their case files
- Enters client data into Efforts to Outcome (“ETO”), HMIS data system and other databases as required by Interfaith and funders in a timely and accurate manner
- Provides services in the community; working with clients in various settings, which include but are not limited to, other agency programs, faith centers, and school administrations
- Participates in all staff meetings and trainings
- Performs additional duties as assigned
EXPERIENCE / EDUCATION
- Bachelor’s Degree in Social Services or related field; or equivalent combination of education and experience preferred
- 2+ years of relevant work experience required
- Experience working with homeless and/or vulnerable populations preferred
- Experience working Seniors preferred
- Bilingual (Spanish/English) preferred
