We're looking for someone to join our team as a Clinic Coordinator (Outpatient) (AHCL) who:

Supports outpatient clinic physician and interdisciplinary teams in facilitating patient care with the underlying objective of enhancing the quality of clinical outcomes and patient satisfaction. Maintains responsibility for implementing day-to-day care management interventions for identified clinic patients. Coordinates small program(s) with limited budget/impact.

ESSENTIAL FUNCTIONS:

  • Coordinates and monitors patient flow and staff activities to ensure prompt, courteous and accurate response to patients. Ensures effective clinic organization through planning, leading, controlling and organizing clinic activities.
  • Works collaboratively and maintains active communication with physicians, nursing and other members of the multi-disciplinary care team to affect timely, appropriate patient management. Utilizes advanced conflict resolution skills as necessary to ensure timely resolution of issues.
  • Conducts telephonic initial and follow-up assessment of clinic patients. Participates in program evaluation and quality improvement activities.
  • Performs other job-related duties as assigned.

You'll be successful with the following qualifications:

Education:

  • High School Education/GED or equivalent: Required
  • Associate’s/Technical Degree or equivalent combination of education/related experience: Preferred

Work Experience:

  • Five years' technical experience: Preferred

Required License/Certifications:

  • Medical Assistant (MA) certification: Preferred

The estimated base pay for this position is $19.53 to $26.51. Additional individual compensation may be available for this role through years of experience, differentials, extra shift incentives, bonuses, etc. Base pay is only a portion of the total rewards package, and a comprehensive benefits program is available for qualifying positions.
