General Summary: The Community Health Worker has a deep understanding of the community served and will serve as a liaison between community members and health service providers to address COVID-19 vaccine hesitancy in the community by providing culturally appropriate education about the vaccine.

JOB RESPONSIBILITIES:
  • Attend and successfully complete assigned trainings on Community Health Worker core competencies.
  • Provide culturally appropriate health education and resources to the community through in-person interactions, health education materials (print and virtual) and follow up phone calls.
  • Works closely with the PHC CEO to meet program goals.
  • Communicate on a regular basis with community stakeholders, PHC CEO, and others as needed.
  • Develop ongoing professional relationships, rapport, and influence with others in the community.
  • Attend community meetings such as City Council and professionally represent PHC in those community meetings.
  • Assess community needs and assist in matching resources to meet those needs.
  • Travel frequently within Pinal County to provide community outreach to underserved populations to address health disparities.
  • Track number of connections made and attendance at community events.
  • Performs other duties as assigned or necessary as they relate to the general nature of the position.
QUALIFICATIONS:
  • Education – High school diploma or GED, required.
  • Experience – Six months of experience interacting with Pinal County community members or other rural Arizona communities, preferred. Currently living within Pinal County, preferred.
  • Language – Bilingual (Spanish and English) preferred
  • Proof of COVID-19 vaccination required.
Regulatory--
  • Minimum 21 years of age.
  • Arizona Level One Fingerprint Clearance Card (must possess upon hire and maintain throughout employment).
  • CPR, First Aid & AED certification, if required (must possess upon hire and maintain throughout employment).
  • Valid Arizona Driver’s License, 39-month Motor Vehicle Report, proof of vehicle registration and liability insurance that meet company insurance requirements, if required.
  • Negative TB test result, if required (Employer provides).
