POSITION PURPOSE: The Population Department Data Analyst will be responsible for aggregating, disseminating and placing data into a uniform setting. They will also ensure that data integration occurs while providing administrative/clerical support to the Population Health Department. This staff will focus on keeping departmental operations running smoothly and efficiently. Commitment to multitasking to help the department to remain organized and assist in the preparation and review of weekly and monthly reports. Perform various data collection and analysis as needed. Responsible for maintaining an accurate clerical system and performing other duties assigned by the Population Health Department.

POSITION REQUIREMENTS / QUALIFICATIONS:

Education/Experience: High School Graduate or GED required Associates/Bachelors preferred. Must have secretarial/clerical skills and least two (2) years’ work experience in an office environment with data analyst experience. Intermediate to advanced Microsoft Office Suite knowledge. Analytical skill sets, attention to detail and knowledge of E.HR systems preferred.

Licensure / Certification: Maintain current CPR certification from the American Heart Association.

Skills / Ability: Dictation skills; (must be able to take and translate minutes at meetings), computer skills, interpersonal skills. Ability to deal/communicate efficiently, effectively and courteously with a wide variety of individuals. Ability to work independently and posse strong-organizational skills. Strong computer-based skills are a must. Ability to work as a team and independently. Intermediate to Advanced knowledge of Office, Teams and Zoom needed. Bilingual preferred.

POSITION RESPONSIBLITIES (THIS IS A NON-EXEMPT POSITION):

 Responsible for uploading content to the department SharePoint page.

 Responsible for scheduling teleconference meetings for the department.

 Maintains department records.

 Assists in templating activities for the Population Health Department.

 Assist with maintenance of email folders and solution reach messages.

 Prepares and submits agency reports to include operational and clinical data regularly and as needed.

 Identify gaps in the existing reporting and develop and recommend efficient solutions for users.

 Shares data with appropriate staff as necessary to assist with decision making.

 Provides scalable solutions for how data is stored for server-based reporting.

 Must be able to provide database configuration expertise.

 Evaluates system performance and makes recommendations for improvements.

 Collects, coordinates, and prepares the weekly and monthly reports as required by Management.

 Functions as liaison between performance improvement and departments.

 Recommends improvements in Policies/Procedures and scheduling to increase efficiency of departments with Performance Improvement issues.

 Development of presentations and visual aids, as necessary.

 Participates in providing information for Uniform Data Systems Report.

 Participates in providing information for grant reporting, as necessary.

 Prepares agendas, attends meetings, distributes agenda, takes dictations, and transcribes minutes of the meeting into proper format within five working days of meeting; distributes reports of proceedings.

 Assists with typing, gathering, duplicating, and assembling materials to be covered at various meetings.

 Establishes and maintains good interpersonal relationships with patients and staff.

 Maintains open communication using appropriate chain of command regarding departmental issues.

 Maintains open relationship with organizations internal department

 Communicate with ACO to appropriately create spreadsheets and organize data sets

 Access EHR to run QI indicators for distribution

 Making travel arrangements for other company employees’ business trips

 Sorts mail and responding to it or distributing it to appropriate employees and nursing support staff

 Monitors office supplies and reporting when stock levels are low to Population Health Manager for approval prior to ordering

 Handles incoming and outgoing correspondence, forwarding incoming mail to appropriate staff members.

 Performs other duties as assigned.
