Fri 6p-8a, Sat 8p-8a = 26 hours (overnights)
Sun 8a-4p, Wed 6p-8a (overnight), Thurs 6p-8a (overnight) = 36 hours
Mon - Fri 8a-4p = 40 hours
Tue 5p-9p, Wed 5p-9p, Thur 3p-9p, Fri 9a-9p, Sat 12p-8:15p = 34.25 hours
Mon 8a-2p, Weds 8a-2p, Thurs 8a-8p, Sat 8a-8p = 36 hours
Sun 8a-6p, Mon 12p-8p, Fri 11a-8p = 27 hours
Sun 12p-6p, Weds 4p-8a (overnight), Sat 6p-8a (overnight) = 36 hours
Sun 2p-8p, Mon 6p-8a (overnight), Fri 8p-12p (overnight) = 36 hours
Thurs 1p-8p, Fri 10a-8p, Sat 11a-5p = 23 hours
JOB DESCRIPTION SUMMARY:

We are hiring full time and part time Direct Support Professional position with flexible schedules, 11 paid holidays, two weeks vacation, and a comprehensive benefits package options, including but not limited to: health, dental, and vision insurance. 403b Retirement Plan!

ESSENTIAL FUNCTIONS:
- Advocate for assigned individual(s), i.e., attend all meetings, ensure all medical, dental and therapy appointments are scheduled and kept and assist in developing and maintaining family and community relationships.
- Attend to individuals’ personal care needs. (This may include lifting and assisting individuals in and out of wheelchairs, use of Hoyer lift and maintaining good hygiene.)
- Transport individuals, as needed, using personal vehicle (i.e., to appointments, work, etc.)
- Transfer from one assignment to another as requested (e.g., from one residence to another or from day program to residence and vice versa).
RESPONSIBILITIES:
- Assist in developing, implementing and monitoring Individual Program Plans (IPP).
- Maintain accurate records including daily logs, individual files, accident and incident reports.
- Maintain open and focused communication with team members regarding goals and progress of program participants.
- Follow established schedules for both individuals we serve and those set by House Managers for staff. Comply with all policies as stated in the Personnel Policies and Procedures Handbook.
- Be sure any facility where individuals use CW services, (i.e., residences, day program or community -based centers) are kept clean and safe.
- Project professional image at all times.
PHYSICAL TOLERANCES:
- Ability to do standing/sedentary tasks for long periods of time.
- Ability to perform range of motion tasks, including but not limited to; reaching, bending, stretching, stooping and lifting.
- Ability to lift up to 50 pounds (all personnel are trained in correct lifting procedures.)
QUALIFICATIONS:
- High School Diploma.
- Valid Driver’s License
- Satisfactory driving record per agency’s insurance company standards.
- Ability to speak, read and write in English
REPORTS TO: House Manager or designated person
SUPPORTED BY: House Manager, Regional Program Manager, Program Director and Human Resources Manager
JOB CLASSIFICATION: Non-Exempt
