Located at 400 Executive Parkway in Oroville, CA, Prestige Assisted Living at Oroville has 44 apartments of assisted living, as well as 16 units of memory care, for a total of 60 units.

POSITION SUMMARY
The Expressions Director 3 is responsible all aspects of care and programming for the Expressions neighborhood including coordinating resident care, managing health services systems, providing supervision to health services team members and planning, directing, promoting, and implementing Expressions memory care programming.
ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Creates a resident-centered approach to the planning and delivery of services to Expressions residents, supporting residents’ rights to participate in decision-making and treating residents with dignity and respect.
  • Evaluates health, functional, and psychosocial status of Expressions residents. Develops individualized service plans and partners with health services leadership as needed. Proactively manages care and services for each resident, maximizing the resident’s opportunity to remain in the assisted living environment. Problem-solves with community management team to case manage residents changing needs.
  • Evaluates residents with reported changes of condition. Provides team member direction, service planning input, and coordination of care to meet residents changing needs.
  • Provides supervision and leadership to all Expressions team members. Responsible for selection, training, scheduling, coaching, performance management, and annual reviews of Expressions team members.
  • Collaborates with the management team on the move in of prospective residents, taking into consideration the needs of the prospective resident, current community acuity, and staffing levels. Provides in-person pre-move in evaluations, if necessary.
  • Ensures resident care needs are met by coordinating care services with physicians, discharge planners, nurses, home health agency staff, case managers, and state agency personnel. Oversees the arrangement of ancillary services such as pharmacy, podiatry, lab, and x-ray as needed.
  • Communicates regularly with residents’ families regarding health care needs. Provides education and support to residents and families.
  • Provides initial and ongoing training to Expressions team members on various topics including but not limited to personal care, medication management, and Expressions product.
  • Directs team members in the delivery of personal care services utilizing Expressions materials. Utilizes resident’s Life Story information to expand cooperative service plan to provide exceptional personalized care.
  • Partners in the investigation and root cause analysis of incidents. Collaborates with the management team on the development and implementation of appropriate interventions.
  • Oversees the documentation and communication of resident care and services. Ensures resident records are completed in a timely manner and maintained according to company policies and state regulations.
  • Creates and leads monthly programming for the Expressions neighborhood utilizing the Expressions focus plan and product materials. Ensures the daily delivery of life enrichment activities in the Expressions neighborhood incorporating the 5 Signature areas of programming.
  • Collaborates with community leadership to maintain survey and regulatory compliance.
  • Drives financial performance of Expressions neighborhood by effective management of labor and supply budgets against census numbers.
  • Assists in the management of Infection Control. Provides training to team members on infection control practices, investigates, and reports incidents of infection and tracks infection data.
  • Participates in manager on duty rotation providing answers on questions regarding resident concerns and staffing coverage.
  • Participates in leadership team by actively contributing to decision making, incident follow up, building and company-wide initiatives and attending leadership team meetings such as daily stand up, Systems Monitoring and Resident Tracking (SMART), Quality Assurance Performance Improvement (QAPI), and other meetings as required.
  • Acts as an ambassador of Prestige by representing the community in a positive manner to outside agencies and participating in community events and social functions.
  • Performs other duties as assigned.

QUALIFICATIONS AND EXPERIENCE
Minimum 2 years of experience in geriatric care and medication administration. Current First Aid and CPR card required. Previous experience training and staff development, budgets, and team management preferred. Memory care experience preferred. Ability to creatively problem-solve in both resident care and employee management situations.
EDUCATION
Bachelor’s degree or equivalent knowledge and skills obtained through a combination of education, training, and experience.
