The La Pine Community Health Center (LCHC) is located in beautiful Central Oregon and serves the tri-county area of Deschutes, Klamath, and Lake Counties.
LCHC is a Federally Qualified Health Center in La Pine, Oregon. The health center provides primary care services to uninsured, under-insured, Medicare, Medicaid, and commercially insured patients. We provide 360-degree patient care and focus on what is best for the patient and the community we serve. In October 2021, the Health Resources and Service Administration formally recognized LCHC with the CHCR Award! LCHC is now ranked the in the top 10% of all health centers nationwide based on overall clinical performance and quality measures!
La Pine's population of over 1,800, is located about 15 miles south of Sunriver, Oregon and 30 miles south of Bend, Oregon. The health center serves a large geographical area covering southern Deschutes, northern Klamath, and western Lake counties, containing about 25,000 people. The area is renowned for its outdoor recreation opportunities such as snow skiing, water sports, hiking, camping, rock climbing, biking, fishing, and much more. Schedule:
40 hours per week with regular shifts of four (10 hour) or five (8 hour) days per week. Weekly shifts involve 32 patient facing hours and 8 hours of administrative time. All LCHC’s sites are closed on Sundays. On call rotation is every 8 to 10 weeks.

ESSENTIAL DUTIES AND RESPONSIBILITIES
- Provision of comprehensive primary care services for all ages in a primary medical care ambulatory clinic setting. The incumbent works independently and interdependently with other medical providers and visiting specialists.
- Provide care consistent with evidence-based medicine.
- Perform minor outpatient procedures in accordance with the scope of practice and privileging policy.
- Provide consultation to other clinic providers either in person or by telephone.
- Consult with clinic physicians, specialists, or other health care professionals as appropriate.
- Assess patient needs for specialty referrals, diagnostic and in-patient care and make appropriate referrals.
- Provide patients and family members with counseling and education regarding health maintenance, disease prevention, diagnosis, treatment, and need for follow up as appropriate during each patient visit. Verify and document that the patient understands diagnosis, treatment and follow-up recommendations as required by uniform data system, meaningful use and patient centered medical home standards.
- Dispense medications in accordance with State of Oregon Pharmacy Board policies.
Education & Licenses:
- 3+ years as a practicing Family Nurse Practitioner
- Licensed Nurse Practitioner
