TITLE: Registered Nurse – Hospice (Admissions and Visits)

The posted compensation range of $56.95 - $78.30/Hour is a reasonable estimate that extends from the lowest to the highest pay Pathways Home Health & Hospice in good faith believes it might pay for this particular job, based on the circumstances at the time of posting. Pathways Home Health & Hospice may ultimately pay more or less than the posted range as permitted by law.
POSITION SUMMARY:
The Hospice Nurse admits patients to service, assesses the patient’s and family’s physical, psycho-social, environmental, safety and developmental needs. They develop an individualized plan of care based on their assessment and document their observations and interventions related to the patient's environment. This role coordinates care with a multidisciplinary team, participates in the hospice program's quality assessment performance improvement program and conducts follow up visits as needed.
AREAS OF RESPONSIBILITY:
- Coordinates rescheduled visits on weekends and agency holidays.
- Schedules admission and follow up visits to patients as requested by a physician or indicated by patient status change on a weekend or holiday.
- Processes new patient referrals by following established procedures.
- Coordinates field staff and patient visit schedules to ensure timely home visits and adequate staffing for weekend/holiday patient needs.
- Assures that adequate staffing is available for Pathways Kids needs.
- Receives/processes key communication from referral sources including patient information, physician orders and customer service issues.
- Consults with patients and physicians as needed regarding immediate needs and makes recommendations as appropriate for next steps to be taken.
- Participates in producing a work schedule for self, supervisory relief, triage nurses for the weekend, and weekend support personnel.
- Assists to develop and update weekend procedure manual.
- Orients and trains weekend clinical coordinator relief personnel to weekend operations.
- Ensures office and property security by employing normal office safety practices and by keeping up with emergency planning guidelines.
- Participates in quality improvement activities related to areas of practice as appropriate.
- Carries out other related activities as required/requested.
QUALIFICATIONS:
- California Registered Nurse (RN) license required
- Minimum of one year of recent Home Health or Hospice experience required
- Current CPR certification – American Heart Association
- Current California driver’s license and automobile insurance.
