Reliance Hospice located in Palm Desert, CA is seeking qualified Registered Nurse (RN) to join our team. The Registered Nurse (RN) plans, organizes, and directs hospice care and provides professional nursing care by utilizing all elements of the nursing process. Position is Full Time, Monday through Friday 8am-5pm.
Responsibilities:
- Complete initial, comprehensive, and ongoing assessments
- Provide professional nursing care
- Assess and evaluate patient's status
- Collaborate with physicians to create a Plan of Care for each patient
- Counsel patient and family/caregivers in meeting nursing and related needs
- Complete, maintain, and submit accurate and relevant clinical notes in a timely manner
- Communicate with physician regarding patient's needs
- Works in cooperation with the family/caregiver and hospice Interdisciplinary Group Members to meet the emotional needs of the patient and family/caregiver
- Supervises ancillary personnel
- Attends weekly Interdisciplinary Group meetings and in-services
Requirements:
- Current California Registered Nurse (RN) License
- Minimum one year of Registered Nurse (RN) experience
- Maintain CPR certification
- Current Drivers License
- Knowledge of EMR's preferred
- Working knowledge of health care laws and regulations
