Health Quality Innovators is currently seeking an Improvement Consultant who will work with physicians to reduce the prevalence of diabetes in Southern Maryland. In conjunction with several partners, the Improvement Consultant will support National Diabetes Prevention Program referrals by helping to identify applicable primary care practices for program recruitment, conduct recruitment outreach and help secure enrollment in the program. In this role, you will provide onboarding and training support for enrolled practices, be a liaison between enrolled practices and community National Diabetes Prevention Program providers, implement training and technical assistance for identifying patients with prediabetes using electronic health records data, and promote the adoption of clinical protocols for making and tracking National Diabetes Prevention Program referrals. You will apply the latest quality improvement tools and techniques based on evidence-based interventions. You will be part of the region’s most experienced and respected healthcare quality improvement organization, as well as work with esteemed partners to improve care coordination and clinical outcomes.

Applicants must demonstrate the ability to work independently and be part of cross-functional teams, develop strong consultative relationships, manage multiple priorities with tight deadlines, interpret data, and design and implement plans of action using improvement tools and evidence-based interventions (i.e., PDSA, LEAN, etc.). Familiarity with physician practices, CRISP Health Information Exchange, health equity concepts, such as social determinants of health, clinical decision support tools, quality improvement strategies, workflow redesign, and electronic health records highly desirable.

Requirements:

Bachelor’s degree in a health care field or related area and three years of relevant experience required. Master’s degree preferred.

Ability to interpret data, such as performance on benchmarking measures, and provide guidance on identifying and prioritizing opportunities for improvement.

Excellent oral and written communication skills, including the ability to build consultative relationships with primary care practices and/or external stakeholders. Experience coordinating training sessions and providing technical assistance.

Must be able to function effectively as a member of a remote team, manage multiple priorities, and be comfortable using good judgment to keep work moving in the absence of complete information.

Proficient in Office 365 and adept with time management.

Limited travel required as needed within the state/region. Most travel will be day trips within Southern Maryland.
