We currently have a need for home health LPNs and RNs Independent Provider nurses who are:

• certified through Ohio DODD to provide waiver nursing services (LPN or RN),
• or certified Ohio Department of Medicaid nurses to provide PDN (private duty nursing).

Responsibilities:

• Perform patient monitoring.
• Record and report patient vitals and vital signs.
• Perform medication administration and record administration times, amount, route, and frequency.
• Assist with patient's person care which may include bathing, toileting, feeding, grooming.

Many of the individuals need assistance with:

• trach care
• ventilator use
• repositioning
• transferring
• gtube care
• wound care
• exercise

About the job:

• This is a self employment opportunity. You are your own boss, billing DODD or ODM for the hours you work. You would not be employed by Portage DD, or another agency.
• There are a variety of schedules open on first, second, and third shift. You decide what hours you would like to work. Overtime may be possible.
• The ideal candidate has experience in home health care and is comfortable working in the home with the family. This is a position where you'd be working in the house or apartment of the individual receiving services, not in a medical center. There will likely be other family members in the home, but you would only be responsible for providing care to ONE individual, and will not have any other responsibilites there.
• There will be a meet and greet to ensure that everyone is comfortable with each other. We want this to be a good fit for all!
• You will be provided training in whatever areas needed. It's ok if you have not worked with people with trachs/vents/gtubes previously. New nurses are welcome, as well as seasoned nurses looking for a slower pace.
• You will be working with ONE individual, and you'll get to know them and their needs very well. You will become a very important person in the family you are assisting.
• You will have the tools you need to do the job well and to the best of your ability. You will go home at the end of your shift with confidence, knowing that you did your job right. You will not be overworked.
• This is a much less intense job than working in a hospital or nursing home. There will be down time.
• You will not be asked to clean, or care for anyone else than the ONE person whose care you will be responsible for.
• The tools you need to do the job safely will be in place. Safety is our priority.
• WE NEED PEOPLE WHO ARE DEDICATED AND WHO PLAN TO STICK AROUND. We understand that life happens, but these folks need nurses who are not planning to leave in the near future.

• RN rates: $41.33 first hour, $29.84 after that, Overtime base $53.92, $42.48 after that. For an 8 hour shift, you'd bill $250.21 on a normal day.
• LPN rates $35.23 first hour, $24.96 after that. Overtime base $45.62, $35.36 after that. For an 8 hour shift, you'd bill $209.95 on a normal day.
