Full-time (40 hours/ Week) Johnston, RI

General Responsibilities: The Integrated Behavioral Health Care Coordinator is primarily responsible to provide various outreach, support and screening services to patients using the standard screening and outcome measures instruments. The IBH Care Coordinator is accessible and visibly present to members of the primary care and IBH teams during clinic hours.

Specific/Significant Job Responsibilities:

  • Participating in daily medical team huddles, medical staff meetings, multidisciplinary teams, and individual supervision sessions with the Integrated Behavioral Health Director.
  • Completing depression, anxiety, and substance use screenings for the Integrated Behavioral Health program with patients as necessary.
  • Assisting Integrated Behavioral Health Clinicians and Director with tracking and outreach for patients with high scores on clinical screenings needing follow-up appointments.
  • Facilitating primary care provider and patient requests for same-day consultation with the Integrated Behavioral Health team.
  • Communicating any urgent clinical needs/crisis calls from patients to IBH team for immediate assistance.
  • Assisting Psychiatric NP providers with completing phone screening assessments for patients interested in establishing treatment and providing resources for alternative levels of care as needed.
  • Scheduling and confirming new and follow-up IBH appointments. Following up on no-show appointments
  • Assuring availability of patient education and other materials related to IBH team work (community health resources, insurance information, chronic disease management and social determinants of health). Obtaining insurance authorizations as necessary.
  • Assisting/ facilitating BH Link IHP referrals, including monitoring and following up on referrals status. Representing IBH program staff in meetings as directed.
  • Participating in continuing education programs, including one (1) annual training program on compliance with OSHA's Blood borne Pathogen Standard.

Required Qualifications: Bachelor’s Degree in social work, psychology or related field required. Previous work experience or internship in a medical setting, preferably a setting that supports a primary care integration model. Experience serving a population of diverse educations, economic and cultural backgrounds. Experience with case management preferred. CPR Certification. Bilingual is an asset, but is not required.
